Book excerpt: Introduction: The purpose of this study was to evaluate the dental and skeletal treatment effects of maxillary incisor proclination, overbite correction and maxillary dentoalveolar expansion with no planned Class II mechanics on the magnitude of mandibular growth or/and projection in growing patients with Class II Division 2 using Invisalign® clear aligners (Align technology, Santa Clara, CA, USA). Methods: Records of 32 patients (12.86 ± 2.01) with Class II Division 2 dental and Class II skeletal relationship were studied retrospectively. Pre (T1) and Post-treatment (T2) CBCT-generated lateral and posterioanterior cephalograms were retrospectively obtained for the treatment group (n=16) and were compared with the T1 and T2 records of an untreated control group (n=16). Proclination and protrusion of the maxillary incisors, as well as maxillary dentoalveolar expansion with no other Class II mechanics were planned in the treatment mechanics. A customized cephalometric analysis was performed. Unpaired t-test and Analysis of Covariance (ANCOVA) regression models were used to compare the linear and angular measurements at both time points for both treatment and control groups to determine if significant skeletal Class II correction was achieved (p

This lower incisor flaring shows great variability among patients. The purpose of this study was to evaluate the effects of Class II severity, mandibular growth, and occlusal plane rotation on lower incisor flaring in patients treated with the Forsus appliance. Materials and Methods: 121 records of Class II patients treated with the Forsus appliance were analyzed retrospectively. Cephalograms were traced at three time points. ANOVA was used to compare changes in cephalometric measurements over time. Pearson's correlation was used to test for relationships between variables. Multiple regression tested for correlation between multiple variables. Results/Conclusions: Lower incisor flaring during Class II correction with the Forsus appliance was only weakly associated with sex, molar relationship change, and occlusal plane steepening. There was no correlation between the change in lower incisor proclination and mandibular growth.

Book excerpt: Patients with Class II subdivision malocclusions are a challenge for clinicians because re-establishing symmetry in one or both arches is often a treatment goal. In patients with a mandibular skeletal asymmetry, surgery is often one of the treatment options. However, patients may be unwilling to undergo surgery, and other options may have to be considered. The aim of this study was to evaluate the etiology and outcomes of Class II subdivision patients treated at the University of Washington Graduate Orthodontic Clinic from 1995-2011. A search of patients treated between 1995 and 2011 resulted in the identification of 110 consecutively treated Class II subdivision subjects with complete records. Ninety-eight subjects could be classified into 1 of 3 groups, based on midline position and dental/skeletal etiology. Initial and final models were used to measure PAR, midlines, overjet, overbite, and molar position. Initial and final cephalograms were traced and measured. Charts were reviewed for information regarding treatment. Twenty-five percent of the 98 subjects presented with their maxillary and mandibular midlines coincident with the face, and were due to a maxillary dental asymmetry. Another 15% presented with maxillary midlines deviated from facial, and were also due to a maxillary dental asymmetry. About 50% of the subjects presented with mandibular midlines that were not coincident with facial, and most of these patients exhibited some degree of mandibular skeletal asymmetry. Over the past 15 years, treatment strategies employed at the University of Washington indicated trends toward less surgery, extractions, and headgear, and towards more reliance on fixed functional use. Ideal correction of midlines was not always achieved, especially in the case of mandibular skeletal asymmetry, with under-correction occurring more commonly than over-correction. Final PAR scores were comparable, regardless of the origin of the asymmetry or extractions status. Lower incisor proclination was increased when fixed functional appliances were used, as well as when a Class I molar relationship was the target for the Class II side. Conclusions: Class II subdivision malocclusions were grouped into 3 main categories, with the largest category being mandibular asymmetry. Interesting trends were noted with regard to treatment strategies, midline and molar correction, and lower incisor proclination.

Book excerpt: The purpose of this study was to retrospectively assess the long term dental and skeletal treatment and growth changes in growing patients with Class II malocclusions treated with the van Beek Headgear Activator (vBHGA) appliance followed by comprehensive fixed orthodontic appliance treatment. A retrospective chart review was undertaken on 40 consecutively treated subjects who were then compared to matched growing untreated control subjects from the Burlington Growth Study and growth forecast simulations. Lateral cephalometric radiographs were used to determine dental, skeletal and growth changes between each treatment phase. Phase 1 vBHGA appliance treatment produced skeletal and dental Class II correction via restraint of maxillary anterior growth, increased mandibular anterior growth, counter clockwise palatal plane rotation, retroclination and retraction of the upper incisors, and proclination and protrusion of the lower incisors resulting in reduced overjet and overbite. The favourable skeletal and dental changes from vBHGA treatment were maintained after the completion of the second treatment phase. The vBHGA appliance was effective in correcting Class II malocclusions by producing favourable skeletal and dental treatment effects in both jaws that were maintained after the completion of phase 2 treatment, in stark contrast to published randomized clinical trials. Success with the vBHGA and potentially other growth modification appliances could be related to patient compliance, timing of the individual's growth spurt, length of phase 1 treatment, and continued orthopedic retention between the first and second phases of treatment.
