Indeed, heated debates have emerged when scholars in social development, social welfare and social policy conducted more systematic comparative research related to the diverse policy measures adopted by Asian governments: which welfare models or typologies best describe Asian cases after the 2008 global financial crisis?; how can contemporary social policy transformations in Asia be appropriately conceptualized?; are particular ‘best practice’ examples evolving in Asia and if so, can they be successfully transferred to enhance social welfare governance among Asian economies? This book combines contributions that address Asian government responses in the light of the above questions. This book explores the daily realities of working life for middle managers in the UK’s National Health Service during a time of radical change and disruption to the entire edifice of publicly-funded healthcare. It is an empirical critique of the movement towards a healthcare model based around HMO-type providers such as Kaiser Permanente and United Health. Although this model is well-known internationally, many believe it to be financially and ethically questionable, and often far from 'best practice' when it comes to patient care. Drawing on immersive ethnographic research based on four case studies – an Acute Hospital Trust, an Ambulance Trust, a Mental Health Trust, and a Primary Care Trust – this book provides an in-depth critical appraisal of the everyday experiences of a range of managers working in the NHS. It describes exactly what NHS managers do and explains how their roles are changing and the types of challenges they face. The analysis explains how many NHS junior and middle managers are themselves clinicians to some extent, with hybrid roles as simultaneously nurse and manager, midwife and manager, or paramedic and manager. While commonly working in ‘back office’ functions, NHS middle managers are also just as likely to be working very close to or actually on the front lines of patient care. Despite the problems they regularly face from organizational restructuring, cost control and demands for accountability, the authors demonstrate that NHS managers – in their various guises – play critical, yet undervalued, institutional roles. Depicting the darker sides of organizational change, this text is a sociological exploration of the daily struggle for work dignity of a complex, widely denigrated, and largely misunderstood group of public servants trying to do their best under extremely trying circumstances. Over a period of two years, Riccucci traveled specifically to eleven different cities, and from interviews and a large national survey, she gathered quantitative results from cities in such states as New York, Texas, Michigan, and Georgia, that were selected because of their range of policies, administrative structures, and political cultures. General welfare data for all fifty states is included in this rigorous analysis, demonstrating to all with an interest in any field of public administration or public policy that management does indeed matter.

Book excerpt: Welfare reform was a spectacular success in New York under Mayor Giuliani despite the city's history of liberal social programs and its huge, entrenched welfare system. The city reduced the numbers on welfare from 1,120,000 to 460,000 by changing the organizational culture, protecting against fraud, insisting on 'work first, ' adapting information technology, and contracting for job placement. The organizational culture was transformed by bold leadership that changed the welfare agency's mission and goals, overcame internal resistance, and prevailed over politicians who had a vested interest in the status quo and the media that were opposed to welfare reform. Welfare fraud was largely eliminated by dropping from the rolls those who were working and could not appear for in-person interviews, by fingerprinting recipients to catch those enrolled under multiple identities and those receiving welfare checks from other jurisdictions, by uncovering hidden income, by enrolling new applicants only after thorough investigation, and by tightening controls to prevent fraud by corrupt employees. JobStat, a computer-based system modeled after the Police Department's system used to track precinct activity, was developed to track the status of welfare recipients and to monitor the performance of the 'Job Centers, ' which were formerly called welfare offices. JobStat focused the attention of department personnel on performance indicators rather than on minutely specified rules. The Giuliani administration's major contribution to national welfare reform was the creation of the only system in the country with large-scale, alternative work arrangements that was able to acculturate large numbers of the never-employed to the world of work.
