Book excerpt: Cardiac surgery is performed on hundreds of thousands of patients a year, and can have an important beneficial impact on the outcomes of patients with coronary and valvular heart diseases. Despite the favorable recovery of most patients, some will have their post-operative period interrupted by the development of atrial fibrillation, with a host of potential complications including stroke. High risk subgroups may develop atrial fibrillation in more than half of cases, and often despite aggressive prophylactic measures. Treatment of atrial fibrillation and its aftermath can also add days to the hospital stay of the cardiac surgical patient. In an era of aggressive cost cutting and optimization of utilization of health care resources, the financial impact of this arrhythmic complication may be enormous. Experimental studies have led to a greater understanding of the mechanism of atrial fibrillation and potential precipitating factors in the cardiac surgical patient. Prophylactic efforts with beta-blockers, antiarrhythmic drugs and atrial pacing are being used, or are being investigated in clinical trials. New methods of achieving prompt cardioversion with minimal disruption of patient care, and prevention of the thromboembolic complications of atrial fibrillation, are also important therapeutic initiatives. This text is designed to aid health care professionals in the treatment of their patients in the recovery period after cardiac surgery, and to instigate additional research efforts to limit the occurrence of, and the complications following, this tenacious postoperative arrhythmia.

It is associated with postoperative complications including increased risk of stroke, prolonged hospital stay and increased costs. The study consisted of three randomized, prospective trials. In the first study intravenous metoprolol showed to be more effective than oral metoprolol in the prevention of AF after cardiac surgery. In the second study intravenous hydrocortisone reduced the risk of postoperative AF by 37% compared with placebo. In study three intravenous metoprolol therapy showed to be as effective as intravenous amiodarone in the prevention of postoperative AF after cardiac surgery.

Book excerpt: Postoperative atrial fibrillation (POAF) is the most common complication following cardiac surgeries. It occurs in nearly 25% to 50% of patients that undergo coronary artery bypass graft surgery (CABG), valve surgery or a combination of both. POAF is associated with an increased risk of adverse outcomes that include stroke, heart failure, mortality and morbidity (Omae and Kanmura, 2012). It is also associated with longer stays in the intensive care unit (ICU), longer overall hospital stay and increased costs of postoperative care (Echahidi et al, 2008). The aim of this project is to determine if metoprolol or amiodarone is better at preventing or reducing the occurrence of POAF, and to determine if effective monitoring using electrocardiographic (EKG) monitors will enable nurses to rapidly recognize POAF so that immediate treatment can begin so adverse outcomes can be prevented. The findings reflect that metoprolol is the first line drug of choice for treatment and prevention of POAF; however, amiodarone is also recommended to be used in combination with metoprolol for those patients that are deemed to be high risk patients (Ho and Lewis, 2010). Amiodarone is also the first line drug of choice for those patients that are unable to take beta-blockers such as metoprolol (Ho and Lewis, 2010). Findings also reflect that EKG monitoring is an effective means of rapid recognition of rhythm changes. It has been shown that through effective EKG monitoring the development of POAF is able to be recognized at the earliest stage which allows for immediate treatment (Drew et al, 2009). This in turn helps to prevent adverse outcomes from occurring in addition to being able to convert the heart back into a normal rhythm.
