Book Management of Diabetes in the Long term Care Population

Download or read book Management of Diabetes in the Long term Care Population written by Canadian Agency for Drugs and Technologies in Health and published by . This book was released on 2013 with total page 9 pages. Available in PDF, EPUB and Kindle. Book excerpt: The economic, social and health burden of diabetes is significant, especially in the elderly population. This is a common disease that affects more than 20% of adults aged 65 to 75 years and 40% of adults older than 80 years. The prevalence of diabetes in long-term care (LTC) facilities is similar to that in the general population, but may be underestimated. Previous studies indicated that in a Canadian long-term care facility, a screening program reclassified a third of residents as having diabetes during a three-year period. Management of diabetes in LTC facilities is challenging due to the under-resourced environment and the nature of this specific population (e.g. age, higher rates of premature death, functional disability, and more co-morbidities than non-diabetics). Besides lifestyle modification and pharmacological therapy, blood glucose monitoring (levels of blood glucose or HbA1c) is also essential to ensure satisfactory glycemic control. Due to the metabolic differences of diabetes in the elderly compared with younger patients, the approaches of disease management should be different. The burden of rigourous near normal glycemic control (e.g. cost of patient discomfort) may outweigh the benefits (such as preventing long-term complications) in this population. Previous clinical practice guidelines have suggested that the glycemic target should be individualized based on the patient's functional status: patients with good functional status should be maintained at HbA1c lower than 7%, whereas those who are frail (have multiple co-morbidities, increased risk of hypoglycemia or have a life expectancy of fewer than 5 years) should be maintained at a less stringent target of lower than 8%. CADTH published a report in 2010 for the purpose of summarizing guideline recommendations regarding the management of diabetes in a long-term care population, but limited evidence was found. The purpose of this review is to identify any evidence and recommendations noted in the evidence-based guidelines for blood glucose monitoring in diabetic patients in LTC setting.

Book Managing Diabetes and Hyperglycemia in the Hospital Setting

Download or read book Managing Diabetes and Hyperglycemia in the Hospital Setting written by Boris Draznin and published by American Diabetes Association. This book was released on 2016-05-20 with total page 338 pages. Available in PDF, EPUB and Kindle. Book excerpt: As the number of patients with diabetes increases annually, it is not surprising that the number of patients with diabetes who are admitted to the hospital also increases. Once in the hospital, patients with diabetes or hyperglycemia may be admitted to the Intensive Care Unit, require urgent or elective surgery, enteral or parenteral nutrition, intravenous insulin infusion, or therapies that significantly impact glycemic control (e.g., steroids). Because many clinical outcomes are influenced by the degree of glycemic control, knowledge of the best practices in inpatient diabetes management is extremely important. The field of inpatient management of diabetes and hyperglycemia has grown substantially in the last several years. This body of knowledge is summarized in this book, so it can reach the audience of hospitalists, endocrinologists, nurses and other team members who take care of hospitalized patients with diabetes and hyperglycemia.

Download or read book Cognitive Aging written by Institute of Medicine and published by National Academies Press. This book was released on 2015-07-21 with total page 300 pages. Available in PDF, EPUB and Kindle. Book excerpt: For most Americans, staying "mentally sharp" as they age is a very high priority. Declines in memory and decision-making abilities may trigger fears of Alzheimer's disease or other neurodegenerative diseases. However, cognitive aging is a natural process that can have both positive and negative effects on cognitive function in older adults - effects that vary widely among individuals. At this point in time, when the older population is rapidly growing in the United States and across the globe, it is important to examine what is known about cognitive aging and to identify and promote actions that individuals, organizations, communities, and society can take to help older adults maintain and improve their cognitive health. Cognitive Aging assesses the public health dimensions of cognitive aging with an emphasis on definitions and terminology, epidemiology and surveillance, prevention and intervention, education of health professionals, and public awareness and education. This report makes specific recommendations for individuals to reduce the risks of cognitive decline with aging. Aging is inevitable, but there are actions that can be taken by individuals, families, communities, and society that may help to prevent or ameliorate the impact of aging on the brain, understand more about its impact, and help older adults live more fully and independent lives. Cognitive aging is not just an individual or a family or a health care system challenge. It is an issue that affects the fabric of society and requires actions by many and varied stakeholders. Cognitive Aging offers clear steps that individuals, families, communities, health care providers and systems, financial organizations, community groups, public health agencies, and others can take to promote cognitive health and to help older adults live fuller and more independent lives. Ultimately, this report calls for a societal commitment to cognitive aging as a public health issue that requires prompt action across many sectors.

Book Implementation of a Diabetes Management Protocol for Long Term Care dwelling Older Adults

Download or read book Implementation of a Diabetes Management Protocol for Long Term Care dwelling Older Adults written by Deborah A. Broadley and published by . This book was released on 2011 with total page 0 pages. Available in PDF, EPUB and Kindle. Book excerpt: "The goal of this practice change project was to increase provider compliance with evidence-based guidelines for managing long term care-dwelling older adults with diabetes mellitus. Studies have consistently demonstrated diabetes management of this population is suboptimal for reasons including a lack of provider compliance with evidence-based guidelines. However, current guidelines are based on studies that incorporate few if any older adults. Integrating appropriate evidence-based guidelines into electronic health records has been shown to increase provider compliance with those guidelines. The practice change project was selected in response to these concerns. The project incorporated developing an evidence-based protocol for managing long term care dwelling older adults with diabetes mellitus. The protocol was developed in collaboration with the medical staff and the information technology department of the participating facility. It was formatted and integrated into the facility's existing electronic health record (EHR). The change project was implemented in a facility that is the largest provider for geriatric care across the continuum in the state of Connecticut. A pilot project was conducted on three of the facility's long term care units. Fifty-two eligible patients were identified. Six providers were responsible for caring for these patients. All providers were educated on the guidelines as well as the protocol which allowed for individualizing the overall plan of care within the suggested evidence-based guidelines. Process indicator measures were selected to evaluate baseline and outcome data. Provider compliance improved by 43-87 percentage points in all outcome process indicator measures. These results support previous literature findings documenting increased provider compliance with evidence-based guidelines incorporated into electronic health records. A major implication of this project is its potential for positively improving patient outcomes by serving as a model for developing other evidence-based guidelines for managing chronic diseases in older adults." -- Abstract.

This book was released on 2001-02-27 with total page 344 pages. Available in PDF, EPUB and Kindle. Book excerpt: Among the issues confronting America is long-term care for frail, older persons and others with chronic conditions and functional limitations that limit their ability to care for themselves. Improving the Quality of Long-Term Care takes a comprehensive look at the quality of care and quality of life in long-term care, including nursing homes, home health agencies, residential care facilities, family members and a variety of others. This book describes the current state of long-term care, identifying problem areas and offering recommendations for federal and state policymakers. Who uses long-term care? How have the characteristics of this population changed over time? What paths do people follow in long term care? The committee provides the latest information on these and other key questions. This book explores strengths and limitations of available data and research literature especially for settings other than nursing homes, on methods to measure, oversee, and improve the quality of long-term care. The committee makes recommendations on setting and enforcing standards of care, strengthening the caregiving workforce, reimbursement issues, and expanding the knowledge base to guide organizational and individual caregivers in improving the quality of care.

Book Integrated Diabetes Care

Download or read book Integrated Diabetes Care written by David Simmons and published by Springer. This book was released on 2016-10-31 with total page 257 pages. Available in PDF, EPUB and Kindle. Book excerpt: Integrating care across disciplines and organisations around the needs of the person with diabetes has been proposed as an approach that could improve care while reducing cost- but has it and can it? Integrated Diabetes Care- A Multidisciplinary Approach collates evidence of worldwide approaches to both horizontal integration (across disciplines) and vertical integration (across organizations) in diabetes care and describe what was done, what worked and what appeared to be the barriers to achieving the goals of the programmes. Evidence is sought from groups who have developed different approaches to integrating diabetes care in different health systems (eg insurance vs tax payer funded, single vs multiple organization, published vs unpublished). A final chapter brings the evidence together for a final discussion about what seems to work and what does not.

Book Retooling for an Aging America

Download or read book Retooling for an Aging America written by Institute of Medicine and published by National Academies Press. This book was released on 2008-08-27 with total page 316 pages. Available in PDF, EPUB and Kindle. Book excerpt: As the first of the nation's 78 million baby boomers begin reaching age 65 in 2011, they will face a health care workforce that is too small and woefully unprepared to meet their specific health needs. Retooling for an Aging America calls for bold initiatives starting immediately to train all health care providers in the basics of geriatric care and to prepare family members and other informal caregivers, who currently receive little or no training in how to tend to their aging loved ones. The book also recommends that Medicare, Medicaid, and other health plans pay higher rates to boost recruitment and retention of geriatric specialists and care aides. Educators and health professional groups can use Retooling for an Aging America to institute or increase formal education and training in geriatrics. Consumer groups can use the book to advocate for improving the care for older adults. Health care professional and occupational groups can use it to improve the quality of health care jobs.

Book The Use of Diabetic Support Groups by Nursing Home Patients

Download or read book The Use of Diabetic Support Groups by Nursing Home Patients written by Julia Reschetnikow and published by . This book was released on 2013 with total page 96 pages. Available in PDF, EPUB and Kindle. Book excerpt: Diabetes Mellitus is becoming a disease that is increasingly prevalent today. The intent of this project is to offer the healthcare community a program to better educate the elderly diabetic population, and offer them support services during the course of their treatment in a long-term nursing facility. This program is typically aimed at long term nursing centers as a facilitator to diabetes management. Each year in the United States, the costs of diabetes and other disease-related illnesses are escalating. The costs are primarily borne by the elderly population due to deteriorating circumstances and limited access to treatment options. The significance of this program is that in the long run, along with education, support and guidance, the nation may cut the amount of healthcare spending due to geriatric diabetes and related conditions. Utilizing a support system may help guide patients along their treatment trajectories and increase adherence. The methods used for this program began by researching: the diabetic condition, target populations, long-term nursing facilities, support groups, and the projected outlooks for the diabetic condition. The Diabetic Buddy program was created to address patient needs, while increasing knowledge and offering support to diabetic nursing home patients. The findings of the program are: increased acknowledgement about the dangers of diabetes, and decreased complications of diabetes in nursing home patients. The Diabetic Buddy program offers education and support services to geriatric diabetic nursing home patients. Keywords: Health Care Administration, Dr. Gail Young, geriatrics, instruction, social, assistance, chronic illness.
