Book excerpt: Excerpt from Malingering and Feigned Sickness That much of the phraseology is of a more or less popular nature arises from the fact that all the reports quoted were written for public bodies or insurance companies, and this also is why such simple tests as Romberg's are explained. The profession will, I hope, pardon these elementary definitions found in the histories of several cases, for they were interpolated solely to make the reports intelligible to the layman to whom they were addressed. Further, I hope the lapses into non technical language will be excused by medical men when it is pointed out that to some extent this work may be of use to members of the legal profession and those who are responsible for the administration of the National Insurance Act. Estimates of malingering - the wilful, intentional attempt to simulate or exaggerate illness in the pursuit of a consciously desired end - vary greatly, despite the fact that malingering is believed to contribute substantially to fraudulent health care and social welfare costs. There is little consensus about what would constitute a coherent assessment of malingering, and base rates have been difficult to establish. Malingering remains a difficult attribution to make not least since it falls outside the remit of the formal psychiatric classifications.
