Book excerpt: Malaria is a serious health and economic problem in Africa, afflicting more than half of the continent's population. Although HICV/AIDS, TB and nutritional deficiencies pose major development problems on the continent, the challenges presented by malaria are of a different kind, because the disease is widespread, persistent and grossly underestimated by the general population. The importance of malaria, along with HIV/AIDS and other diseases in the development agenda, is now recognised in Goal 6 of the Millennium Goals. This book provides the evidence required to design and implement malaria control strategies in Africa. The chapters analyse the nexus between poverty and malaria with a focus on policies that can be implemented at various levels of society to fight the disease. Malaria and Poverty in Africa is the outcome of a research sponsored by the African Economic Research Consortium (AERC). Augustin Kwasi Fosu is the Director, Economic Division of the United Nations Commision for Africa, Addis Ababa, and former Director of Research at the AERC, Nairobi. Germano Mwabu is Professor of Economics at the University of Nairobi. He was co-director of the AERC project on Poverty Income Distribution and Labour Markets in Sub-Saharan Africa.

Today, however, these drugs no longer work against the deadliest form of malaria that exists throughout the world. Malaria deaths in sub-Saharan Africaâ€"currently just over one million per yearâ€"are rising because of increased resistance to the old, inexpensive drugs. Although effective new drugs called "artemisinins" are available, they are unaffordable for the majority of the affected population, even at a cost of one dollar per course. Saving Lives, Buying Time: Economics of Malaria Drugs in an Age of Resistance examines the history of malaria treatments, provides an overview of the current drug crisis, and offers recommendations on maximizing access to and effectiveness of antimalarial drugs. The book finds that most people in endemic countries will not have access to currently effective combination treatments, which should include an artemisinin, without financing from the global community. Without funding for effective treatment, malaria mortality could double over the next 10 to 20 years and transmission will intensify.

Book excerpt: Filmer empirically explores the relationship between household poverty and the incidence and treatment of fever - as an indicator of malaria - among children in Sub-Saharan Africa. He uses household Demographic and Health Survey data collected in the 1990s from 22 countries in which malaria is prevalent.The analysis reveals a positive, but weak, association between reported fever and poverty. The geographic association becomes insignificant, however, after controlling for the mother's education. There is some evidence that higher levels of wealth in other households in the cluster in which the household lives are associated with lower levels of reported fever in Eastern and Southern Africa. Poverty and the type of care sought for an episode of fever are significantly associated: wealthier households are substantially more likely to seek care in the modern health sector. In Central and Western Africa those from richer households are more likely to seek care from all types of sources: government hospitals, lower-level public facilities such as health clinics, as well as private sources. In Eastern and Southern Africa the rich are primarily more likely to seek care from private facilities. In both regions there is substantial use of private facilities - use that increases with wealth. Like the incidence of fever, treatment-seeking behavior is strongly associated with the level of wealth in the cluster in which the child lives.This paper - a product of Public Services, Development Research Group - is part of a larger effort in the group to understand the relationship between poverty and health. As a Liberian, remembering my days of combat with the disease inspired me to do a global review of the disease with concentration in my home country. This was also based on the fact that Liberia being a war-torn country for more than a decade did not have the proper support to fight the disease nor suppress its widespread.

Malaria impairs economic growth and human development in many of the World Bank's client countries, particularly in Sub-Saharan Africa. The Bank has the capacity to do a lot more than it has in malaria control, including financing, policy advice, and implementation support. It will support countries to develop and implement programs to (i) cost-effectively reduce morbidity, productivity losses in multiple sectors, and mortality due to malaria, particularly among the poor and among children and pregnant women; and (ii) address the challenges of regional and global public goods.

After many years in decline, the continent's economy picked up in the mid-1990s. Along with this macroeconomic growth, people became healthier, many more youngsters attended schools, and the rate of extreme poverty declined from 54 percent in 1990 to 41 percent in 2015. Political and social freedoms expanded, and gender equality advanced. Conflict in the region also subsided, although it still claims thousands of civilian lives in some countries and still drives pressing numbers of displaced persons. Despite Africa’s widespread economic and social welfare accomplishments, the region’s challenges remain daunting: Economic growth has slowed in recent years. Poverty rates in many countries are the highest in the world. And notably, the number of poor in Africa is rising because of population growth. From a global perspective, the biggest concentration of poverty has shifted from South Asia to Africa. Accelerating Poverty Reduction in Africa explores critical policy entry points to address the demographic, societal, and political drivers of poverty; improve income-earning opportunities both on and off the farm; and better mobilize resources for the poor. It looks beyond macroeconomic stability and growth—critical yet insufficient components of these objectives—to ask what more could be done and where policy makers should focus their attention to speed up poverty reduction. The pro-poor policy agenda advanced in this volume requires not only economic growth where the poor work and live, but also mitigation of the many risks to which African households are exposed. As such, this report takes a "jobs" lens to its task. It focuses squarely on the productivity and livelihoods of the poor and vulnerable—that is, what it will take to increase their earnings. Finally, it presents a road map for financing the poverty and development agenda.

This paper evaluates the economic benefits of extending the World Health Organization's DOTS Strategy (a multi-component approach that includes directly observed treatment, short course chemotherapy and several other components) as proposed in the Global Plan to Stop TB, 2006-2015. The authors use a model-based approach that combines epidemiological projections of averted mortality and economic benefits measured using value of statistical life for the Sub-Saharan Africa region and the 22 high-burden, tuberculosis-endemic countries in the world. The analysis finds that the economic benefits between 2006 and 2015 of sustaining DOTS at current levels relative to having no DOTS coverage are significantly greater than the costs in the 22 high-burden, tuberculosis-endemic countries and the Africa region. The marginal benefits of implementing the Global Plan to Stop TB relative to a no-DOTS scenario exceed the marginal costs by a factor of 15 in the 22 high-burden endemic countries, a factor of 9 (95% CI, 8-9) in the Africa region, and a factor of 9 (95% CI, 9-10) in the nine high-burden African countries. Uncertainty analysis shows that benefit-cost ratios of the Global Plan strategy relative to sustained DOTS were unambiguously greater than one in all nine high-burden countries in Africa and in Afghanistan, Pakistan, and Russia. Although HIV curtails the effect of the tuberculosis programs by lowering the life expectancy of those receiving treatment, the benefits of the Global Plan are greatest in African countries with high levels of HIV.
