Book excerpt: In 1957 or thereabouts I became impressed with how immunity could be looked at as a process of Darwinian selection amongst the circulating lymphoid cells of the body. The clonal selection theory which grew out of this has been generally accepted in principle by immunologists, but I do not feel that its full implications in relation to pathology have yet been widely realized. In a previous book, Immunological Surveillance, I have tried to apply the approach to cancer immunity. This is a basically similar attempt to look at auto-immune disease from the same Darwinian point of view. Anyone who attempts to produce acceptable general statements about complex biological and clinical phenomena must have a certain sense of guilt. No biological phenomenon can ever be completely, or even adequately, described. There can, at best, only be a progressive improvement in the acceptability, the intellectual elegance, or the practical usefulness of the working generalizations that can be produced. Any attempt to write at this interpretative level can easily be brushed aside as superficial, unnecessary, and liable to be proved wrong or irrelevant by new developments. It is anathema to many good professional scientists and their objections are real enough.

One or more autoantibodies may be produced by a person's immune system when it fails to distinguish between "self" and "non-self" proteins. Usually the immune system is able to discriminate by recognising foreign substances (non-self) and ignoring the body's own cells (self), yet not overreact to non-threatening substances such as foods, dust and pollen, or beneficial microorganisms. It creates antibodies only when it perceives what it has been exposed to as a threat (non-self). When the immune system ceases to recognise one or more of the body's normal constituents as "self", it may produce autoantibodies that attack its own cells, tissues, and/or organs, causing inflammation and damage. The causes of this inappropriate action are varied and are not well understood, often resulting in a chronic auto-immune disorder. While there is not a direct link, it is thought that many cases of autoantibody production are due to a genetic predisposition combined with an environmental trigger (such as a viral illness or a prolonged exposure to certain toxic chemicals). Some families have been shown to have a high prevalence of auto-immune conditions; however, individual family members may have different auto-immune disorders or may never develop an auto-immune condition. Researchers believe that there may also be a hormonal component to the cause as many of the auto-immune conditions are more common in women of childbearing age. This book presents leading research from throughout the world.

It is evident that such disorders, occurring without any obvious external cause, raise the very elementary question of how immune processes directed against self constituents could be initiated. Generally, of course, the immune system acts as a regulatory and defence mechanism, and disorders of auto-immunity represent breakdowns in this regulatory system. The following chapters will be concerned with the individual components ofthe endocrine system so affected by auto-immune processes; it will first be necessary to provide an initial chapter for the purpose of summarizing some general principles of immunology, in order to place the immune disorders of the endocrine system in context.
