Book excerpt: Les travaux en neuropsycholinguistique, notamment dans le cadre de la maladie d'Alzheimer (MA) qui constitue l'objet de ce travail, décrivent les performances en dénomination dans un cadre essentiellement nominal ou bien en privilégiant le niveau syntaxique.La présente étude propose d'évaluer les performances lexicales de patients avec MA dans une tâche de dénomination d'action en étudiant leurs profils du point de vue de: a) la hiérarchisation du lexique (usages de verbes génériques - enlever / usages de verbes spécifiques - éplucher) et b) la production de paraphasies sémantiques (usages de verbes conventionnels vs non conventionnels).Notre étude est conduite auprès de 8 sujets âgés déments diagnostiqués malades Alzheimer au stade léger et compare leurs performances avec celles de 8 sujets adultes appariés en âge, sexe et niveau socioculturel. Le protocole expérimental utilisé s'inscrit dans le cadre d'une recherche interdisciplinaire (CLLE - ERSS et INSERM U 825) et consiste en la présentation de 17 vidéos d'actions suivie d'une tâche de dénomination d'action (qu'est-ce qu'elle a fait la dame ?). Les résultats statistiques suggèrent, à ce stade débutant de la MA, une probable préservation des compétences lexico-sémantiques pour le traitement des verbes. Toutefois, l'analyse qualitative du type d'erreurs indique la présence de troubles sémantiques fms qui pourraient contribuer à un marquage linguistique de la MA. Ce travail tente d'apporter des éléments d'éclairage sur les atteintes du système sémantique des verbes dans le cadre de la MA et propose de valoriser les compensations langagières du sujet Alzheimer pour améliorer sa prise en charge orthophonique.

Book excerpt: Si l'existence de troubles lexico-sémantiques dans le cadre des premiers stades de la maladie d'Alzheimer n'est plus contestée, les manifestations de ces troubles restent à ce jour controversées. Les effets de catégorie en défaveur des entités biologiques, des verbes ou des items concrets ont fait l'objet de travaux aux conclusions contradictoires, ce qui laisse supposer que les conclusions tirées ne sont que des artefacts des méthodes employées. La batterie d'évaluation des traitements lexico-sémantiques utilisée dans cette étude réunit des épreuves équilibrées en fonction de variables psycholinguistiques et de modalités d'administration. Nous avons comparé les scores de 6 patients Alzheimer à ceux d'une population témoin de 134 sujets, dont 40 ont été évalués dans le cadre de ce travail, afin de déterminer un profi1lexico-sémantique de la maladie d'Alzheimer. Sur un plan strictement quantitatif, nous avons pu mettre en évidence un déficit des patients Alzheimer au niveau du traitement visuel des images, du système sémantique, un effet de régularité pour la lecture et l'écriture ainsi qu'un effet de catégorie en défaveur des entités biologiques, données en parfaite cohérence avec la plupart des travaux réalisés dans ce domaine. Sur un plan plus qualitatif, les signes cliniques de la maladie d'Alzheimer ont été relevés pour chacune des épreuves de notre protocole. Notre étude permet de mieux contribuer au diagnostic de la maladie d'Alzheimer, et ouvre des perspectives pour une prise en charge' orthophonique plus adaptée.

Le chapitre sept est consacré à une analyse des critères fournis pas les sujets selon une approche psycholinguistique. Les trois chapitres suivants sont consacrés à l'analyse linguistique des manifestations dans le discours des processus cognitifs révélés par les analyses psychologiques et psycholinguistiques. Le chapitre onze consiste en une synthèse et une discussion des principaux faits empiriques dégagés des chapitres précédents. L’analyse des résultats ne montre aucun effet de la catégorie sémantique, ni dans le groupe total, ni lorsque l’on considère les deux sous-groupes ayant un degré de sévérité sémantique léger et sévère. En revanche, l’analyse statistique a mis en évidence un effet net de l’interaction sujet/objet au détriment des items ayant une interaction sujet/objet faible, quelque soit la catégorie sémantique. Enfin, l’étude de cas multiples auprès de 3 patients DS a mis en évidence des difficultés lexico-sémantiques très importantes mais des profils hétérogènes. Un seul patient présente un effet catégoriel en faveur des items manufacturés (de faible interaction). Un effet d’interaction sujet/objet n’est retrouvé que chez 2 patients.

La richesse sémantique résiduelle peut-elle s'évaluer au travers des comportements dénominatifs ? Pour répondre à cette question, 12 sujets contrôles et 5 patients présentant une maladie d'Alzheimer ou une aphasie primaire progressive sont soumis aux épreuves de la BETL. L'étude de leur production orale s'effectue à travers une grille analytique des stratégies dénominatives. Dans un premier temps, l'analyse des compétences sémantiques résiduelles dans différentes modalités permet de déterminer le statut sémantique de ces patients. Ensuite, la richesse lexico-sémantique résiduelle est envisagée grâce à une analyse sémique des stratégies compensatoires. Les résultats montrent que cliniquement, les sujets présentant une atteinte du système sémantique s'appuient sur des éléments descriptifs ou autobiographiques, tandis que les sujets présentant un trouble d'accès au lexique phonologique de sortie formulent significativement plus de traits sémantiques pertinents. Les comportements dénominatifs se révèlent donc essentiels à analyser dans les épreuves langagières, afin de proposer une prise en charge plus spécifique des aspects lexico-sémantiques du langage.
