Book excerpt: Since the start of the COVID-19 pandemic in early 2020, decision-makers in affected countries have acted quickly to address the immediate health effects of the pandemic and to put into place public health and social measures to slow or stop the spread of COVID-19. In order to preserve the gains made in maternal, newborn, child and adolescent health in the last decades and avoid negative impacts due to the pandemic, the global community quickly mobilized to advocate for protecting the health of these populations. Since May 2020, the World Health Organization (WHO), through its headquarters, regional and country office teams, has supported 19 countries in five WHO Regions (see Figure 1) to raise the profile of and commitment to maternal, newborn, child and adolescent health and ageing (MNCAAH)b through an Initiative on mitigating the indirect impacts of COVID-19 on MNCAAH services. The goal was to ensure that during the response to COVID-19, actions would be taken to mitigate indirect effects on MNCAAH due to disruptions to service provision and use.

Book excerpt: The COVID-19 pandemic has caused unprecedented disruption worldwide highlighting once again the interdependency of health and socioeconomic development, and the global lack of health systems resilience. Two years into the pandemic, most countries report sustained disruptions across service delivery platforms and health areas with a profound impact on health outcomes. The impact of these disruptions is magnified within marginalized communities and in countries experiencing protracted conflict. There is an urgent need to focus on recovery through investment in the essential public health functions (EPHFs) and the foundations of health systems with a focus on primary health care, and whole-of-government and -society engagement. The aim of this Research Topic is to gather, transfer and promote operationalization of key experiences from COVID-19 to inform global and country level recovery that better promote health; guide policy direction towards building health systems resilience; and thereby ensure economic and social prosperity. Experience with COVID-19 has demonstrated that traditional approaches to health system strengthening have failed to achieve the complementary goals of Universal Health Coverage (UHC) and health security with the divide between the most vulnerable and well-off only widening. Much of what had been learned from previous experiences such as Ebola in West Africa has not been widely applied. This has left health and economic systems vulnerable to 21 st century public health challenges, ranging from conflict and natural disasters to aging demographics and rising rates of non-communicable and communicable diseases and antimicrobial resistance. These challenges require intentional focus and investment as well as whole-of-government and -society engagement with health to build health system resilience. Greater action is needed to prevent the devastating effects of war and conflict on the health of the most vulnerable. This Research Topic will convene the knowledge and practices of leaders in public health, health systems, and humanitarian and development sectors. This is to ensure lessons from COVID-19 inform the recovery agenda and promote sustainable health and socioeconomic recovery for all. Lest we forget and find ourselves again unprepared and vulnerable in the face of an even greater threat.

This work addresses an identified gap in measurement and monitoring of health system resilience. It complements the Health Systems Resilience Toolkit and supports implementation of the recommendations in WHO’s position paper on building health system resilience for UHC and health security. The package aims to support countries to progressively build their capacities to measure, monitor and build health system resilience from national to subnational levels covering health facilities and other service delivery platforms. It emphasizes an integrated approach to health system strengthening underpinned by essential public health functions, encompassing health emergency preparedness. It includes: - guidance on how to utilize and adapt the health system resilience indicators, including a step-by-step guide - a suite of recommended health system resilience indicators with technical specifications - supplementary indicators of relevance to health system resilience The primary target audience for this package is national and subnational health authorities (including planners and managers) and service providers, as well as local, regional, and global technical organizations and partners working on health system strengthening, including WHO, United Nations country teams, donors, nongovernment organizations, development and humanitarian agencies, and other health-related technical agencies.

WHO estimates that 58 million people had chronic hepatitis C virus (HCV) infection globally in 2019, and less than a quarter of them were diagnosed. New and innovative approaches are needed to accelerate progress toward the HCV elimination targets. Self-testing is one such approach. These guidelines provide a new recommendation and guidance on HCV self-testing to complement existing HCV testing services in countries. These guidelines also highlight operational considerations to support strategic implementation and scale up of HCV self-testing.

Book excerpt: These consolidated guidelines on HIV prevention, testing, treatment, service delivery and monitoring bring together existing and new clinical and programmatic recommendations across different ages, populations and settings, bringing together all relevant WHO guidance on HIV produced since 2016. It serves as an update to the previous edition of the consolidated guidelines on HIV. These guidelines continue to be structured along the continuum of HIV care. Information on new combination prevention approaches, HIV testing, ARV regimens and treatment monitoring are included. There is a new chapter on advanced HIV disease that integrates updated guidance on the management of important HIV comorbidities, including cryptococcal disease, histoplasmosis and tuberculosis. The chapter on general HIV care, contains a new section on palliative care and pain management, and up to date information on treatment of several neglected tropical diseases, such as visceral leishmaniasis and Buruli ulcer. New recommendations for screening and treating of cervical pre-cancer lesions in women living with HIV are also addressed in this chapter. Guidance on service delivery was expanded to help the implementation and strengthening the HIV care cascade. Importantly, this guidance emphasizes the need for differentiated approaches to care for people who are established on ART, such as reduced frequency of clinic visits, use of multi-month drug dispensing and implementation of community ART distribution. The adoption of these efficiencies is essential to improve the quality of care of people receiving treatment and reduce the burden on health facilities, particularly in resource limited settings.

Four departing members were appreciated for their contribution to the STAG-NTD. The next (seventeenth) meeting of the STAG-NTD will be held in 2023.

The report examines the effect of the COVID-19 pandemic on access to NCD medicines, and the policies and strategies implemented by countries and health systems to anticipate and mitigate stresses across NCD medicine supply chains. The full range of upstream and downstream impacts are investigated, including: manufacturing; procurement, importation and last mile delivery; patient-level effects through affordability and availability; and the effects on NCD medicine availability by category of disease. The report culminates in recommended actions and interventions for key stakeholders in the NCD pharmaceutical supply chain, including governments, regulatory authorities, manufacturers and the private sector; as well as directions for future research for improving access and supply chain access resilience.
