The main attention so far has been focused upon organ metastases. The molecular mechanisms thereof, while far from being totally elucidated, are increasingly well understood. Modern diagnostic tools now enable detection and precise localization of small lesions. In contrast, our knowl edge and diagnostic capabilities regarding metastatic spread to the'lymphatic system are rather limited. However, there have re cently been a number of interesting advances. It is the aim of this volume to submit these developments to detailed analysis. The therapeutic relevance of enhanced sensitivity in detection of lymph node metastases has to be considered in the light of in creased morbidity versus eventual prognostic improvements by modification of therapy. An interesting concept which might im prove diagnostic accuracy while reducing operative morbidity is the "sentinel node" technique. Surgical standards are lacking, however, and many fundamental questions pertaining to precise lymphatic mapping remain unanswered. Whether the sentinel node concept is of general relevance or whether it is applicable in only a few organs remains to be determined. This book offers intensive discussion of the concept from methodological and tu mor biological viewpoints. World-renowned experts with long term involvement in related basic and clinical research provide the reader with a broad survey of actual knowledge together with a critical appraisal of recent and future developments. Book excerpt: The partition of fluid between the vascular and interstitial compartments is regulated by forces (hydrostatic and oncotic) operating across the microvascular walls and the surface areas of permeable structures comprising the endothelial barrier to fluid and solute exchange, as well as within the extracellular matrix and lymphatics. In addition to its role in the regulation of vascular volume, transcapillary fluid filtration also allows for continuous turnover of water bathing tissue cells, providing the medium for diffusional flux of oxygen and nutrients required for cellular metabolism and removal of metabolic byproducts. Transendothelial volume flow has also been shown to influence vascular smooth muscle tone in arterioles, hydraulic conductivity in capillaries, and neutrophil transmigration across postcapillary venules, while the flow of this filtrate through the interstitial spaces functions to modify the activities of parenchymal, resident tissue, and metastasizing tumor cells. Likewise, the flow of lymph, which is driven by capillary filtration, is important for the transport of immune and tumor cells, antigen delivery to lymph nodes, and for return of filtered fluid and extravasated proteins to the blood. Given this background, the aims of this treatise are to summarize our current understanding of the factors involved in the regulation of transcapillary fluid movement, how fluid movements across the endothelial barrier and through the interstitium and lymphatic vessels influence cell function and behavior, and the pathophysiology of edema formation. At present there are no models for direct measurement of tumor cell spread by lymphatics. This is needed for understanding basic biology of lymphatic involvement developing treatments and assessing their effectiveness. Purpose: To develop and assess IVVM for imaging the morphology and fluid dynamics of lymphatic vessels in mouse models during tumor development. Scope: Assessment of the concept that in vivo video microscopy (IVVM) can be used for studying experimental cancer spread via the lymphatic system. Methods: Primary tumors were formed by intradermal injection of cancer cells (ventral midline between inguinal lymph nodes). IVVM was used to examine lymphatics on one side (skinflap under anesthesia, 1-25 days post injection); conventional histology used to examine other side. Results: Tumor formation was accompanied by a well developed lymphatic network adjacent to the tumor and enlarged lymphatic vessels with valves in tissues between the tumor and lymph nodes as seen by histology and lVVM. By IVVM, lymphatic endothelium and cells carried by lymph flow could be clearly seen. Conclusion: IVVM is a powerful tool for studying cancer cells as they interact with the lymphatic system.
