Whilst the vast majority of biochemical tests assess the level of constituents in blood and urine at a given point in time, an impor tant additional parameter is the assessment of physiological reserve function by means of loading tests, prOlonged fasting, exercise and clearance studies. The protocol and interpretation of response of each of these stress tests form the main basis of the text. An attempt has been made to be reasonably comprehensive in the range of tests described; hence, not only have some rather older ones been re tained, but also some very recently introduced ones included. It is appreciated that practice varies widely from laboratory to labora tory with respect to protocol, sample collection, methodology and quotation of reference ranges, and no doubt also from clinician to clinician, particularly regarding interpretation of results. In paedi atric work, micromethods will demand much smaller volumes of blood than those stated here, which relate generally to adult medi cine. The account, therefore, must be seen as a guide to practice rather than as a recipe; it is in no wayan attempt to standardise procedures, which depend on the local requirements.

Book excerpt: Gonadotropin-releasing hormone (GnRH) cells are the key regulators of reproductive function in all vertebrate organisms. The GnRH molecule is synthesized in a small number of neurons in rostral hypothalamic regions of the brain. In mammals, these neurons release the GnRH decapeptide into the portal capillary system leading to the anterior pituitary gland. There, GnRH causes the release of the gonadotropins, luteinizing hormone (LH) and follicle-stimulating hormone (FSH), which in turn act upon the gonads to stimulate their maturation, and to cause synthesis of sex steroid hormones, estrogen, progesterone and testosterone. Although each of the levels of the hypothalamic-pituitary-gonadal axis is critical for reproductive function, GnRH neurons play the primary role in the control of reproductive maturation and adult reproductive function, and may even play a role in reproductive senescence. Since its discovery in 1970, there has been intense interest in GnRH-producing neurons, with more than 8000 papers and chapters in the last decade alone. Despite this activity of research in basic and clinical science, there has never been a book written specifically on GnRH neurons. GnRH: The Master Molecule of Reproduction aims to bring together the large and diverse literature of both laboratory and applied research that focuses on these unique cells. This book will provide basic background into reproductive neuroendocrinology, as well as specifics regarding the role of GnRH neurons in the control of reproduction. Students studying endocrinology, reproduction, neuroendocrinology or molecular endocrinology will benefit from this book. In addition, this book will take a multi-species approach which will be useful both to basic researchers as well as clinicians. We have witnessed a veritable explosion of chemical, physiologic and pharmacologic data on this hypothalamic peptide and the approximately 1500 agonist and antagonist analogs that have been synthesized. In order to track this expanding field, I was asked to organize an international symposium on basic and clinical aspects of LHRH analogs as part of the Reproductive Health Care: CDS Symposium held in Maui, Hawaii, in October 1982. This meeting brought together a number of the leading investigators in the field. Much new state-of-the-art information was presented which I and my colleagues felt deserved a wider audience. Drs Vickery, Nestor, and Hafez consented to undertake this task. Upon review of the literature, it was apparent that there was no recent text which fully covered the breadth of developments in the field. Accordingly, the editors decided to use the symposium as a nucleus on which to build a singular, comprehensive state-of-the-art analysis of this rapidly growing discipline, and the application of such knowledge to reproductive medicine. As exemplified by the various areas of expertise provided by the individual contributors, it becomes obvious that the scope of the subject matter, while relating solely to a well-defined chemical class (LHRH analogs) and a circumscribed physiologic and pharmacologic entity (reproduction), has expanded enormously.
