Urinary tract infections are the number one problem for the elderly in long term care facilities. Research evidence supports improved infection control as the most effective form of treatment rather than antibiotic use, however antibiotic treatment continues and is on the rise creating antibiotic resistance, increased costs and resources or hospitalizations. By searching urinary tract infections in elderly living in long term care facilities or nursing homes, antibiotic use in elderly as well as signs and symptoms in elderly with urinary tract infections is how evidence was obtained for this proposal. It is in the best interest for quality patient care to re-educate staff in the process of infection control to decrease the risk of urinary tract infections. This proposal will include a description of the problem a description of the solution an implementation plan, an evaluation plan, dissemination plan, review of literature as well as educational material for health care staff for improved infection control in long term care facilities.

Book excerpt: "According to CDC, urinary tract infection (UTI) is among the most prevalent healthcare associated infections, contributing to 20% to 30% of all infections affecting nursing home residents. Older adults in the nursing homes are prone to developing infections due to their compromised immune system as a result of advanced age and other multiple medical conditions. One clinical practice site identified a need to reduce urinary tract infection (UTI).. The EBP change project included an educational intervention on providing effective urinary catheter and meatal care through the utilization of effective hand hygiene practices. This intervention was based on Agency for Healthcare Research and Quality (AHRQ) safety program for long term care facilities in reducing healthcare acquired catheter associated infections. The AHRQ Catheter Care and Maintenance Questionnaire was to evaluate improvement in participants knowledge. As a result of the implementation of the educational intervention, there was a decrease in UTI rate from 4.5% (n=9) to 2.5% (n=5); foley catheter usage rate decreased from 7% (n=14) to 4.5% (n=9). And an overall improvement in participant's knowledge. This paper will discuss the design, implementation and evaluation of an EBP change project. " -- Abstract

Book excerpt: Background: Urinary tract infection (UTI) is common, often over-diagnosed, in long-term care (LTC) facilities and a source of inappropriate antibiotic prescription. Objectives: (1) Establish factors associated with a nurse's decision to send a urine culture (UC), and if those factors were associated with a positive culture result; (2) to determine if antimicrobial therapy is associated with functional improvement. Method(s): 101 LTC residents were prospectively identified and assessed after submission of a urine specimen for culture. Logistic regression was performed to identify variables associated with a positive culture result. Result(s): Change in behaviour, dysuria, and change in character of urine were the three main reasons for UC collection. Male sex and change in mental status were the only significant predictors of culture positivity. Treatment did not lead to significant improvement in ADL score. Conclusion: UCs obtained from LTC residents is often not appropriate and antibiotic treatment of residents from whom urine specimens are obtained does not lead to functional improvement.

Book excerpt: A retrospective study was done in a long-term care facility based on secondary analysis from existing charts (n=40) to identify the population's beverage intake and presence of a facility-acquired urinary tract infection. The study described the amount of caffeinated beverages intake and occurrence of facility acquired urinary tract infections in a 144-bed skilled nursing facility that offers long-term care. The study was guided by the conceptual framework of the Health Promotion Model (HPM). The research study compared the rates of urinary tract infections in the selected population based on the frequency of caffeinated beverages consumed on an average daily basis. A retrospective chart review was conducted including an analysis of the daily frequency of caffeinated beverage intake and the occurrence of a facility-acquired urinary tract infection during the past three months. A week's worth of data was used to determine each participant's average daily caffeinated drink consumption. Descriptive statistics and an independent sample test were used to generate the answer to the research question. The statistical results yielded that there is no significant difference between the amount of caffeinated beverage intake among the residents with UTI and those without UTI. The amount of caffeinated beverages intake appears to be a non-significant of UTI in this facility. Caffeine in its relation to urinary tract infections has not been studied much at this point in time. Because of the lack of research on this subject, it would be beneficial for future research to be done concerning this issue. Further research concerning prevention of urinary tract infections in the long-term care resident population, in general, would also be beneficial due to the rising costs of health care and quality of life for the resident.
