Book excerpt: In Latin America and the Caribbean, more than 8 million older people are unable to independently complete at least one basic activity of daily living, such as eating, bathing or showering, or getting in and out of bed. This situation, called care dependence, affects 12% of people over age 60 and nearly 27% of people over age 80 in the region. The demand for long-term care services will more than triple in the next 30 years, due primarily to population aging and the epidemiological transition. The region will also see a drop in the traditional supply of long-term care services, which are based mainly on the unpaid work of women in families. Given this outlook, societies and governments must ready themselves to support care-dependent people and their caregivers, while at the same time advancing gender equity. Countries need to design long-term care systems, and they need to do so now. This publication addresses five key questions for countries in the region that wish to start this process: 1) Why is long-term care an issue that cannot wait?; 2) How do countries design a long-term care system?; 3) What are the countries in the region doing?; 4) How much would a long-term care system cost?; 5) Where should we start? People who are chronically ill, have serious disabilities, have HIV/AIDS, are mentally ill, or victims of accidents and disasters, or elderly - many will need continuing care and support and these numbers will grow. How best to meet these needs is getting more attention. Such care is not to just look after the sick but to enable those with long term illnesses or disabilities to live their lives as fully as possible. Institutionalization is often not the best way of care and the home where the patient lives with family members and friends nearby is often more appropriate. This report examines the options, highlighting the clear benefits of home-based care whilst being aware of the needs of the carers in the home. The report stresses it is time for health systems to take responsibility for providing caregivers in families and communities with the support they both need, and to bring greater benefit to the patient.

Book excerpt: Over the last two decades, many changes have happened to the social welfare policies of various industrial countries. Citizens have seen their pensions, unemployment benefits, and general healthcare policies shrink as “belt tightening” measures are enforced. But in contrast, long-term care has seen a general growth in public financing, an expansion of beneficiaries, and, more generally, an attempt to define larger social responsibilities and related social rights. The aim of this book is to describe and interpret the changes introduced in long-term care policies in Western Europe. The volume argues that recent reforms have brought about an increasing convergence in LTC policies. Most of the new programs have developed a new general approach to long-term care, based on a better integration of social care and health care. The book explores increasing public support given to family care work (in the past, the family would take care of the elderly or infirm) and increasing growth and recognition of a extended social care market (by which care has shifted from a moral obligation based on family reciprocity to a paid, professional activity). A new social care arrangement has therefore been developing in Western countries, based on a new mix of family obligations, market provision, and public support. In order to understand such changes, this analysis will take into account the social and economical impact of these reforms.

Countries take a universal or selective approach, or some mix of the two, in identifying the population that will be covered under LTC programmes. Decisions are based on the economic and social contexts, and LTC is designed with consideration of the existing infrastructure for delivering health and social care. Universal approaches are grounded in the principle of ensuring equal access to health and social care. Selective approaches focus on those in greatest need, primarily those who are low-income. While selective approaches are perceived to cost less, the total costs may be offset by the high cost of implementing means-testing to identify beneficiaries. The selective approach may also result in high levels of unmet needs among people who do not meet the low-income thresholds. Mixed universal and selective approaches enable universal coverage for some services or populations and means-tested eligibility for others; thus, older people may face high payments for some needed services. Many LMICs have initiated mixed approaches to LTC, using public and private resources while building the policies, systems and infrastructure for universal coverage of LTC.

Book excerpt: Long-term care can be vexing on a personal as well as social level, and it will only grow more so as individuals continue to live longer and the population of aged persons increases in the United States and around the world. This volume explores the ethical issues surrounding elder care from an ecological perspective to propose a new theory of global justice for long-term care. Care work is organized not just nationally, as much current debate suggests, but also transnationally, through economic, labor, immigration, and health policies established by governments, international lending bodies, and for-profit entities. Taking an epistemological approach termed “ecological knowing,” Lisa A. Eckenwiler examines this organizational structure to show how it creates and sustains injustice against the dependent elderly and those who care for them, including a growing number of migrant care workers, and how it weakens the capacities of so-called source countries and their health care systems. By focusing on the fact that a range of policies, people, and places are interrelated and mutually dependent, Eckenwiler is able not only to provide a holistic understanding of the way long-term care works to generate injustice but also to find ethical and practicable policy solutions for caring for aging populations in the United States and in less well-off parts of the world. Deeply considered and empirically informed, this examination of the troubles in transnational long-term care is the first to probe the issue from a perspective that reckons with the interdependence of policies, people, and places, and the first to recommend ways policymakers, planners, and families can together develop cohesive, coherent long-term care policies around the ideal of justice.
