Book excerpt: Nearly seventy percent of cancer patients receive radiotherapy (RT) during the course of treatment making it one of the most widely employed anti-cancer therapies. The efficacy of RT is believed to reside in the capacity of radiation to mediate lethal DNA damage to tumor cells leading to tumor regression. In addition to the direct effects of radiation in mediating DNA damage, radiation can augment the capacity of tumor cells to interact with, and be recognized by, the host immune system. In particular, we demonstrate that endogenous CD8+ T cells play an essential role in the efficacy of local RT across several murine tumor cell types with a wide range of intrinsic radiosensitivities. Treatment of established tumors with local radiation increased tumor infiltration by T cells that was associated with increased expression of local T cell attractive chemokines. In addition, T cell infiltration was biased towards CD8 + T cells which altered the proportions of T cell subsets in the local microenvironment to generate a favorable effector:suppressor T cell ratio. Importantly, local RT did not increased the systemic pool of tumor antigen-specific CD8+ T cells and was unable to mediate a significant abscopal effect against a distant untreated tumor in a model of disseminated disease. Combination local RT and systemic immunotherapy could further enhance the local effect. In an effort to understand the mechanisms of tumor control following treatment with local RT, we identified type I interferon (IFN) as an essential component in the host immune response. The efficacy of local RT was dramatically diminished when host responsiveness to type I IFN was abolished, and bone marrow derived hematopoietic cells are demonstrated to be the essential targets of host type I IFN. The local accumulation of myeloid derived suppressor cells (MDSCs) was dramatically increased in tumors arising in type I IFN nonresponsive hosts and was further augmented by local radiation leading to exaggerated local immune suppression. Therapeutic depletion of MDSCs in type I IFN nonresponsive hosts was sufficient to restore CD8 + T cell mediated tumor regression following treatment with local RT. Collectively, our results suggest that the major function of local radiation with regard to host immunity is to increase effector T cell infiltration into the local tumor microenvironment generating a favorable environment for immune-mediated tumor control. In addition, host type I interferon is essential for limiting the local accumulation of immune suppressive myeloid cells therefore fostering a tumor microenvironment suitable for T cell mediated tumor regression. This work begins to unravel the complex interplay between conventional cancer therapy and host immunity that will likely provide a groundwork for the rational combination of radiotherapy and immunotherapy.

In order to assure aperfect balance within the enormous com plexity of the immune system, it is not surprising that multiple self-regulatory mechanisms are organized at different levels, such as antibody feedback, idiotypic-anti-idiotypic responses, suppres sor and helper T cells, lymphokine signals and genetic require ments. A nu mb er of observations in recent years have, however, demonstrated that consistent contributions to the immunological homeostasis are given also by signals generated outside of the immune system, namely,in the central and autonomous nervous system as weIl as in the endocrine apparatus. Furthermore, the interactions between the immune system and the other body homestatic mechanisms seem to be bidirectional: if immunological cells may be targets of neuroendocrinological factors, immunological products seem in turn to contribute to the neuro endocrine homeostasis.

Book excerpt: For long, high dose ionizing radiation was considered as a net immune suppressing agent, as shown, among others, by the exquisite radiosensitivity of the lymphoid system to radiation-induced cell killing. However, recent advances in radiobiology and immunology have made this picture more complex. For example, the recognition that radiation-induced bystander effects, share common mediators with various immunological signalling processes, suggests that they are at least partly immune mediated. Another milestone was the finding, in the field of onco-immunology, that local tumor irradiation can modulate the immunogenicity of tumor cells and the anti-tumor immune responsiveness both locally, in the tumor microenvironment, and at systemic level. These observations paved the way for studies exploring optimal combinations of radiotherapy and immunotherapy in order to achieve a synergistic effect to eradicate tumors. However, not all interactions between radiation and the immune system are beneficial, as it was recognized that many of radiation-induced late side effects are also of immune and inflammatory nature. Currently perhaps the most studied field of research in radiation biology is focused around the biological effects of low doses, where many of the observed pathophysiological endpoints are due to mechanisms other than direct radiation-induced cell killing and are immune-related. Finally, it must not be forgotten that the interactions between the ionizing radiations and the immune system are bi-directional, and activation of the immune system also influences the outcome of radiation exposure. This Research Topic brings together 23 articles and aims to give an overview of the complex and very often contradictory nature of the interactions between ionizing radiation and the immune system. Due to its increasing penetrance in the population both through medical diagnostic or environmental sources or during cosmic travel low dose ionizing radiation exposure is becoming a major epidemiological concern world-wide. Several of the articles within the Research Topic specifically address potential long-term health consequences and the underlying mechanisms of low dose radiation exposure. A major intention of the Editors was also to draw the attention of the non-radiobiological scientific community on the fact that ionizing radiation is by far more than purely an immune suppressing agent.
