Book excerpt: This volume reflects, in part, an update of Clinical Application of Neuropsycho logical Test Batteries, edited by Theresa Incagnoli, Gerald Goldstein, and Charles Golden some 10 years ago. While the initial concept of the present editors involved doing a straightforward update of each chapter, it soon became apparent that the field of clinical neuropsychology had changed so dramatically and rapidly that substantial changes in the outline had to be made. It was our view that sufficient interest remained in the standard comprehen sive neuropsychological test batteries to make an update worthwhile. We asked four senior people to take on this assignment, James Moses, Jr. , andAmold Purisch in the case of the Luria-Nebraska Battery, and James Reed and Homer Reed for the Halstead-Reitan Battery. These individuals all have long-term associations with these procedures and can be viewed as pioneers in their development. How ever, it also seemed to us that there was an increasing interest in the psychometric aspects of the standard procedures and in assessment issues related to the relative merits of using standard or individualized assessment strategies. Thus, we have chapters by Elbert Russell and Gerald Goldstein that provide discussions of these current methodological and clinical issues. During the past 10 years, the cognitive revolution has made a strong impact on neuropsychology. The interest of cognitive psychologists in brain function has increased dramatically, and we now have an active field of cognitive neuropsy chology, something that was only beginning 10 years ago.

This has been the result of increased recognition by psychologists of the potential role they can play in evaluating patients involved in lawsuits and the increasing sophistication of lawyers who have recognized that damages can go beyond claims of physical and motoric impairment. However, this increase in involvement has not always been matched by a more sophisticated recognition of how the client (whether plaintiff or defendant) can best be served by the neuropsychologist. I have personally seen or reviewed many cases in which the psychologists involved did not effectively present their case. This partially occurred because they failed to recognize the difference in presentations aimed at clinical audiences and those aimed at a legal proceeding. It also occurred because they failed to recognize that the standard of proof necessary is very different in a hospital and in a courtroom. Finally, it occurred because they rarely recognized what the unique role of the psychologist can be in either countering or supporting the testimony of medical specialists. The purpose of this volume is to bring some light on these questions. The presentations here are write-ups of the work presented at the Third Annual Conference on the Luria-Nebraska Neuropsychological Battery held in Omaha during May, 1985. All of the papers emphasize the role of the Luria-Nebraska but the ideas can be used with almost any test.

The approach is illustrated using the computer-assisted cognitive remediation program developed by the author (NeurXercise). Twenty-nine different cases are presented in depth, following the outline presented in the earlier portions of the book. Treatment planning emphasizes Luria’s concepts of alternate functional systems and double dissociation. Pre- and post-treatment neuropsychological testing results are presented along with ecological validity and generalization to daily functioning. The cases are presented by etiology and each section concludes with what can be learned about that etiologic process from the cognitive remediation results. This section includes blast injuries, which are still being characterized in the literature, as well as effective treatment of patients with head injury, stroke, post-neurosurgical sequelae, co-morbid disorders, subcortical disorders, attention deficit disorder, and learning disabilities. As a result of the extensive amount of activity that neuropsychology has experienced recently, two major developments have emerged. First, several approaches have evolved regarding comprehensive neuro psychological assessment. There are presently several standard test bat teries in common use, as well as an approach to assessment that does not make use of standard batteries, but rather fits the evaluation accom plished to the particular diagnostic problems presented by the indi vidual patient. Second, a great deal of specialization has emerged, with assessment procedures developed for evaluation of specific types of neuropsychological deficit. The purpose of this volume is to review these developments, particularly with reference to their implications for application in clinical settings. The history of this book's development is of particular interest. Some years ago, the Veterans Administration became concerned with developing an optimal method of neuropsychological assessment for its many health-care related facilities. Initially, the problem was con ceptualized in terms of whether the VA should encourage wider use of the long-established Halstead-Reitan Neuropsychological Test Battery (HRB) or promote more extensive use of the recently developed Luria Nebraska Neuropsychological Test Battery (LNNB). A conference was funded to bring together authorities in clinical neuropsychology to dis cuss this matter and present a series of papers to invited psychologists from various VA facilities.

Does it not make both conceptual and practical sense to identify an individual's relative strengths and weaknesses so that abilities or strengths can be utilized to optimize functioning in areas of deficit? Although these all sound like reasonable precursors to assisting individuals in need of academic assistance, there are those who would argue otherwise. Impor tantly, however, the Director of the National Institutes of Health, in a recent report to Congress, advocates exactly what this volume proposes. Indeed, there are important needs to be addressed, and the literature reviewed herein strongly supports the conclusions drawn by these authors.

Book excerpt: It is a privilege to be asked to write the foreword for so excellent a book, so timely and so much needed by the field. Not only is it most unusual these days to have a single authored volume on so broad a topic, but Dr. La Rue has done a superb job of providing both a scholarly treatise and a practical handbook. With a burgeoning elderly population and the corresponding increase in geriatric psychopathology, the needs of mental health services are exceeding by far the supply of appropriate providers. In an effort to meet this need, psychiatry, medicine, neurology, pharmacology, psychology, nursing, and social work have all made the provision of training in geriatrics and gerontology a high priority-but I fear we are losing the race. For example, multidisciplinary teams that assess, diagnose, and treat mental health disorders in elderly patients are incomplete without clinical psychologists and neuropsy chologists, and yet there is barely a handful of clinical psychologists trained in dealing with geriatric patients. We can count on our fingers the additional ones graduated each year. In hospitals, clinics, and private practices across the country, otherwise skilled psychologists are unprepared to respond to the special mental health needs of the elderly. A few CME programs are helping to address this need, but they are clearly not enough.
