Book excerpt: Le groupe « médicaments essentiels » de la société nationale française de médecine interne (SNFMI] a publié en 2013 une liste de 100 médicaments essentiels. En juin 2013, ce groupe d'initiative a engagé une large validation de cette liste par une enquête Delphi. Un groupe d'experts a été formé par appel à participation des 980 membres titulaires de la SNFMI, leur demandant de valider les médicaments « leur paraissant essentiels pour traiter en ville et/ou à l'hôpital les pathologies observées en France chez l'adulte, en excluant les vaccins, les traitements destinés à l'hémato cancérologie et les traitements spécifiques des maladies rares ». La liste soumise comportait 87 molécules. Lors du premier tour de validation (selon le consensus formalisé par la Haute Autorité de Santé -HAS-), les experts pouvaient proposer 3 nouvelles molécules, soumises à validation si suggérées par au moins 3 d'entre eux, le groupe d'initiative devant au décours de l'enquête en analyser les résultats avec possibilité d'ajout de molécules manquantes. 101 membres de la SNFMI ont participé. 79.2% ont répondu au 1° tour validant 26 molécules, 15 nouvelles étant soumises à validation. Au 2° tour 90% ont répondu, validant 9 molécules. Au 3° tour, 89% ont répondu validant 27 molécules. Au total 63 molécules ont été validées selon la méthode HAS, 101/102 molécules (87+15) ayant fait l'objet d'un très bon ou bon consensus selon les méthodes Delphi classiques. Après discussion le groupe d'initiative a retenu 61 des 63 molécules validées et une liste complémentaire de 24 molécules. Cette première validation d'une liste de médicaments essentiels en France en montre la faisabilité et la complexité.

Un médicament essentiel est un médicament d'efficacité reconnue, dont la balance bénéfice/risque est favorable et qui fait référence dans sa classe thérapeutique. L'OMS a développé le concept de médicament essentiel dans les années 1970 et a proposé récemment une liste de 360 médicaments. Il n'existe pas de liste concernant spécifiquement la médecine générale en France. L'objectif de cette étude était d'établir une liste française de 100 médicaments essentiels en médecine générale. Une liste de 137 médicaments a été établie par 14 médecins généralistes (MG) ruraux et urbains. Cette pré-liste a été soumise pour validation à 35 MG, représentants les départements universitaires de médecine générale. Afin d'obtenir un consensus, une méthode Delphi a été utilisée. Chaque médicament était côté sur une échelle de 1 à 9. Ceux qui était côtés = 7 par plus de 70% des participants étaient retenus. Les médicaments non validés étaient reproposés dans un nouveau tour incluant également des médicaments proposés par les experts. Les tours se sont succédés jusqu'à validation de 100 médicaments, stabilité des réponses entre 2 tours successifs, ou abscence de réponse des experts. 28 experts, MG en activité ont participé à l'étude. Une liste de 104 médicaments a été validée au terme de trois rondes Delphi, 85 au premier tour, 19 au deuxième tour et un troisième tour a validé la liste dans son ensemble. L'ensemble de ces médicaments était commercialisé depuis plus de dix ans. Ils avaient une efficacité reconnue ; 85 étaient classés en Service Médical Rendu majeur par la Haute Autorité de Santé. Cette liste devra être confrontée à la diversité des situations clinques rencontrées en médecine générale. Sa diffusion pourrait constituer un outil pour l'enseignement et une référence pour la pratique.

Book excerpt: Using national drug arrest data from 1980 to 2007, this report illuminates the persistence and extent of racial disparities in the so-called?war on drugs? in the United States. Although blacks and whites engage in drug offenses at roughly comparable rates, blacks have been consistently arrested for drug offenses at rates that are from 2.8 to 5.5 times higher nationwide than white drug arrest rates. In addition, this report reveals that among individual states, black drug arrest rates in a single year, 2006, ranged from 2 to 11.3 times higher than white rates. Finally, the report shows that since 1980, the preponderance of drug arrests have been for possession, not sales. Millions of Americans have acquired a criminal record because they engaged in the minor non-violent offense of drug possession. Human Rights Watch calls on the United States to revise its drug control policies to reduce reliance on criminal prosecution and address these troubling racial disparities.
