For the last two decades, great ad vances have been made in understanding the pathogenesis of those disorders. Recent studies have revealed that the earliest event in atherogenesis is the adhesion of circulating leukocytes to the vascular endothelial cells and their migration into the subendothelial space. These cells are known to playa central role in the formation of a fatty streak consist ing of lipid-laden foam cells. As pathological events continue, the lesion is converted to a more fibrous lesion associated with vascular smooth muscle cells. To solve the enigma of this complicated process, intensive studies in molecular biology have disclosed the genes involved in those events. Some of the genes have been verified by creation of novel animal models, which have led to novel therapeutic strategies for subjects with atherosclerosis. This volume contains papers presented at the International Symposium on Lipoprotein Metabolism and Atherogenesis held in Kyoto December 5-8, 1998, supported in part by the Japan Intractable Diseases Research Foundation. The following three topics were the focus of the three-day program: I) The molecular approach to studying risk factors and prevention 2) The creation of novel animal models 3) Lipoprotein disorder as a cause of activation of vascular endothelial cells Thirty distinguished researchers from the United States, the United Kingdom, Austria, Finland, Australia, and Japan were invited.

We hope that this volume of edited papers, a consequence of the third international colloquium on atherosclerosis, conducted at the University of Brussels, Belgium through the support of the "Fondation de Recherche sur l' AtherosclE,rose" will contribute to this goal. Among the topics discussed the major ones were the mechanism of action of lipolytic enzymes, the deficiency or dysregulation of cellular receptors, the genetic deficiencies of apolipoproteins, and the panoply of external factors as diet, physical exercise, drugs, which mOdify the lipoprotein metabolism. Special interest was also devoted to potent techniques as kinetic analysis of metabolic tracers and use of monoclonal antibodies. Their contribution to the detection and treatment of atherosclerosis will be obviously essential in the future.

The concept of the "protective" action of HDL was thoroughly explored within the framework of its molecular biology with data on its epidemiological as well as its in vitro mechan ism(s) of action being discussed. The action of drugs on arterial and HDL metabolism was also discussed as were newer aspects of platelet aggregation, especially as related to prostaglandins. New ground was also broken in discussions of lipid mobilization and mechan isms of hypocholesteremia. Cardiovascular disease results from complications of a silent and chronic arterial disease: atherosclerosis. The challenge for the practitioner is adapting diagnostic and therapeutic responses to prevent this common and complex disease. Dyslipidemia, are disorders of the metabolism of soluble transporters of lipids in extracellular spaces of the human body (including blood), called lipoproteins. They are major cardiovascular risk factors, causally related with atherosclerosis and are themselves multifactorial diseases, resulting from interactions between genetic and environmental factors. The study of genetic factors has recently taken a new path with the study of DNA as an experimental object. More than fifty genes of lipoprotein metabolism have been identified in both their physiological actions and their contribution to the pathogenesis of human dyslipidemia. The diversity of observations has refined our current knowledge of the control of lipid metabolism and energy homeostasis in living organisms beyond the limits of the cardiovascular system (e.g., brain, immune system, and development). These studies have given way to a shake-up of former phenotypic classifications, distinguishing new entities, defining targeted therapeutic strategies, providing a basis for different patterns of disease distribution in human populations.
