Book excerpt: Les cauchemars (CM) sont des rêves, dysphoriques, qui génèrent un éveil et dont le souvenir est particulièrement marquant. Ils sont un phénomène commun en population générale. Ils peuvent être traités pas des thérapies cognitives et comportementales (TCC) comme le préconise l'Association Américaine de la Médecine du Sommeil (AASM). En population psychiatrique les CM sont le second trouble du sommeil le plus fréquents. En pratique, les CM sont rarement traités et leurs TCC peu connues donc peu utilisées. Objectif: Dans cette thèse, nous effectuons une revue systématique de la littérature, dans l'objectif d'étudier l'efficacité des différentes thérapies cognitives et comportementales des cauchemars et d'en évaluer l'intérêt dans différents troubles psychiatriques. Méthodes PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) : 316 résultats ont été extraits des bases de données Pubmed, Cochrane, Science Direct et ESBCO jusqu'à juillet 2021. 20 articles ont finalement été inclus dans la revue, dont 14 essais comparatifs randomisés. 3 types de TCC sont ici étudiés: la thérapie d'entraînement à la visualisation (TEV), la TCC de !'Insomnie (TCC-I) et la thérapie d'exposition de relaxation et réécriture (TERR). Les populations incluses souffraient de troubles de stress post-traumatique (TSPT), de troubles dépressifs, de troubles psychotiques et d'idées suicidaires. Résultats : En plus d'une efficacité dans les cauchemars idiopathiques comme post traumatiques. La TEV, la TCC-I et la TERR démontrent une efficacité modérée à large sur l'ensemble des symptômes étudiés : anxiété, syndrome dépressif, insomnie, persécution. Seules les idées suicidaires, présentent des résultats contradictoires. Il n'a pas été trouvé de preuve de supériorité lorsque l'on compare ces thérapies entre elles ou avec les traitements habituels. Discussion: Ces résultats sont prometteurs mais demandent à être confirmés par des essais comparatifs de plus forte puissance.
