Il s'agit là d'un enjeu majeur de santé publique. Face aux nombreux obstacles rencontrés par ces patients, le concept d'unité mobile de soins dentaires semble une alternative intéressante. Le but de cette thèse est de connaitre les moyens qui permettraient la mise en place d'un tel projet dans la région Pays de la Loire. Dans un premier temps, nous exposerons des généralités concernant la santé bucco-dentaire et la prise en charge des personnes en situation de handicap. Dans un second temps, nous réaliserons un recensement des personnes en situation de handicap et des établissements médico-sociaux de la région. Par la suite, nous nous intéresserons plus spécifiquement aux unités mobiles depuis le XVIIème siècle jusqu'à aujourd'hui. Puis, nous nous pencherons sur l'organisation légale d'une structure non fixe. Par ailleurs, nous nous questionnerons sur le financement d'une unité mobile. Enfin, nous énumèrerons quelques difficultés et questions soulevées par la réalisation d'un tel projet qui pourraient freiner sa concrétisation.

À travers différents exemples d'unités mobiles de soins présentes en France et pour diverses populations, l'objectif est de démontrer que la mise en place de ces dispositifs peut représenter une alternative dans le parcours de soins de la population handicapée, afin de faciliter l'accès à la santé bucco-dentaire. La création d'une unité mobile de soins dans la région Pays de la Loire pourrait-elle être envisagée, après avoir évalué les contraintes du cahier des charges et l'ensemble des difficultés auxquelles ce projet serait confronté ?

Dans ce travail nous abordons pourquoi, comment, où et quand cette offre est mise en place en France. Nous soulevons quelques interrogations quant à son développement futur.

Book excerpt: Contexte : En Guyane, la faible démographie de chirurgiens-dentistes présents sur le territoire et la répartition inégale de l'offre de soins sont deux problématiques majeures dans l'accès aux soins bucco-dentaires. Pendant longtemps, les services de téléconsultations étaient peu développés en Guyane. La pandémie de Covid-19 a permis le développement de ces services permettant la continuité et l'accès aux soins, mais ils ne concernent toujours pas l'odontologie. Objectifs : Étudier la connaissance, le recours et l'intérêt des chirurgiens-dentistes guyanais vis-à-vis de la télémédecine bucco-dentaire. Méthodes : Nous avons réalisé une étude transversale auprès des chirurgiens-dentistes inscrits au tableau de l'Ordre Départemental de Guyane par la diffusion d'un questionnaire entre août et septembre 2021. Résultats : Quarante-trois chirurgiens- dentistes guyanais ont répondu au questionnaire. La télémédecine bucco-dentaire était peu utilisée. Cependant, les chirurgiens-dentistes guyanais avaient dans l'ensemble connaissance de cette pratique et la considéraient comme utile, notamment dans la gestion des urgences, la planification des rendez-vous et la consultation de patients à mobilité réduite ou vivants dans des zones éloignées ou des établissements médico-sociaux. Ils étaient une majorité à être intéressés pour l'intégrer à leur pratique. Conclusion : La télémédecine bucco-dentaire apparaît donc comme une solution possible et acceptable pour les chirurgiens-dentistes pour lutter contre les inégalités d'accès aux soins bucco-dentaires en Guyane.
