Comme la température et certaines hormones, le sommeil a donc une caractéristique chronobiologique. Ce sommeil peut souvent être altéré aussi bien dans sa qualité, son rythme, son architecture, sa durée. Il est composé de cycles, eux-mêmes composés de différents type de sommeil (lent ou paradoxal), chacun d'entre eux pouvant être sujet à des troubles. La majorité des troubles psychiatriques comportent des troubles du sommeil plus ou moins spécifiques, et certains troubles du sommeil peuvent engendrer des troubles psychiatriques, notamment thymiques. L'adolescence est une période charnière entre l'enfance et l'âge adulte, et est particulièrement sujette aux troubles du sommeil. On y retrouve des troubles du sommeil spécifiques comme le syndrome de retard de phase, mais également des troubles du sommeil de l'enfant et de l'adulte. De même, certains troubles psychiatriques commencent à l'adolescence, et l'intrication entre troubles du sommeil chez l'adolescent et troubles psychiatriques est particulièrement importante. Il existe également des maladies rares à expression psychiatrique dont une des manifestations principales correspond à un trouble du sommeil (syndrome de Kleine-Levin ou syndrome de Smith-Magenis). Enfin, l'apport de la chronobiologie en psychiatrie est en plein essor, avec par exemple le développement actuel d'antidépresseurs mélatoninergiques. La prise en charge de l'aspect chronobiologique du sommeil peut être un mode de prise en charge des troubles psychiatriques des troubles du sommeil de l'adolescent.

Book excerpt: Résumé : INTRODUCTION : Les troubles du sommeil chez l'adolescent sont fréquents et souvent banalisés. Un quart des jeunes selon le baromètre santé jeunes INPES de 2010 en souffrent. Leurs répercussions à court et long terme ne sont pas négligeables : difficultés scolaires, sociales, maladies psychiatriques et cardiovasculaires. Il n'existe pas de recommandations globales sur la prise en charge diagnostique et thérapeutique de ces troubles. Peu de cours à la faculté y sont consacrés, peu d'études médicamenteuses sur cette problématique existent. L'objectif de cette étude est de comprendre comment sont appréhendés les troubles du sommeil de l'adolescent chez le généraliste, quelles sont les difficultés rencontrées lors de la prise en charge. Puis nous émettrons des voies d'amélioration possibles. MATERIEL ET METHODE : Nous avons réalisé une étude qualitative sous la forme d'entretiens semi dirigés auprès de treize généralistes sélectionnés pour leur expérience présumée sur ce sujet. RESULTATS : Un trouble du sommeil sera rarement un motif de consultation. Banalisé, minimisé voire masqué par les adolescents et/ou parents, ce trouble sera souvent non exprimé et donc à rechercher par le généraliste devant d'autres plaintes. Deux troubles prédominent : des troubles du sommeil comportementaux en lien avec l'évolution de notre société avec les nouvelles technologies informatiques et des troubles du sommeil psychiatriques réactionnels à des événements de la vie. Un interrogatoire exhaustif permet souvent de poser le diagnostic. Le recours au psychiatre peut être parfois nécessaire. De nombreux facteurs sont intriqués dans l'étiologie de ces troubles d'où une certaine complexité et ce d'autant plus que l'adolescent a du mal à se livrer. La correction d'erreurs soit le rétablissement d'une hygiène de sommeil est la première mesure thérapeutique entreprise. En cas d'échec ou en association, des spécialités de phytothérapie sont prescrites mais leurs indications et leur efficacité sont souvent reconnu limitées. L'homéopathie est peu prescrite car souvent méconnue. Les hypnotiques, source de mésusage, de dépendance, sont majoritairement non prescrits ou pour certains seulement de manière encadrée, ponctuelle, dans des indications précises. L'absence de guide thérapeutique, le manque de formation ressenti sur les troubles du sommeil et sur la manière d'aborder un adolescent en consultation constituent des entraves à une prise en charge adaptée d'après les médecins interrogés. De plus, les adolescents peuvent être non compliants, réticents à une prise en charge entre autres psychiatrique ou une prise en charge qui remettrait en cause leur autonomie naissante. CONCLUSION : La promotion du sommeil des adolescents comme un élément clé de leur santé peut être améliorée, que ce soit auprès des adolescents eux-mêmes, parents et médecins. Médecins généralistes, médecins, infirmiers scolaires, enseignants, professionnels de la route, journalistes pourraient y contribuer.
