Traditionnellement on utilisait de l'hydroxyde de calcium, mais actuellement c'est le MTA qui devient l'outil de plus indiqué. Cependant il existe également une technique qui consiste à provoquer la fin de l'édification radiculaire d'une dent permanente immature nécrosée, c'est la revascularisation. La première partie de cette thèse est une analyse bibliographique des différentes techniques d'apexification et de revascularisation de la dent permanente immature nécrosée pour comparer l'efficacité de celles-ci. La deuxième partie est une enquête réalisée auprès des praticiens exerçants en Alsace pour déterminer leurs attitudes thérapeutiques face à ce genre de cas très complexes. Un questionnaire comprenant des cas cliniques munis de photographies et de radiographies leur ont été soumis via une plate-forme internet. L'objectif de cette thèse est d'évaluer la pratique professionnelle au vu des évolutions biologiques et technologiques dans le domaine de la gestion apicale d'une dent permanente immature nécrosée.

En effet, l'édification radiculaire étant arrêtée, l'apex est large et ouvert ce qui rend le traitement endodontique difficile voir impossible. La racine de ces dents a des parois dentinaires très fines et peut être sujet à des fractures. Le pronostic est donc incertain. Actuellement le praticien a recours, le plus fréquemment, à la technique dite "d'apexification". Les biomatériaux de référence sont l'hydroxyde de calcium et le MTA®, plus récemment la BiodentineTM. Cependant, cette thérapeutique a certains inconvénients, notamment la persistance de la finesse des parois radiculaires pouvant avoir pour conséquence la fracture de la racine. Une nouvelle option thérapeutique a été décrite, la revascularisation. L 'objectif est d'utiliser le potentiel des tissus périapicaux de la dent immature pour générer le potentiel des tissus périapicaux de la dent immature pour générer une croissance tissulaire à l'intérieur du canal. Ainsi, elle permettrait la fin de l'édification radiculaire. Cette alternative semble séduisante mais, vu le recul clinique et les différents protocoles existants, elle ne peut pas être considérée comme une donnée acquise de la science. Cette thèse a pour objectif une revue de littérature, d'étudier les avantages, les inconvénients de cette technique mais aussi d'examiner la nature des tissus nouvellement formés.
