Book excerpt: Introduction : Le refus scolaire anxieux, communément appelé "phobie scolaire", regroupe certaines situations où un enfant ou adolescent s'absente et a des difficultés à se rendre à l'école, se manifestant par une simple opposition ou d'importants symptômes physiques pouvant engendrer une déscolarisation totale. Les symptômes somatiques sont à type de désordres digestifs et douleur chronique. Ils sont peu étudiés en pratique quotidienne. Notre objectif était de mieux caractériser ces symptômes dans le refus scolaire anxieux. Matériel et méthodes : Une revue de la littérature a d'abord été menée avec 148 articles inclus (jusque mai 2020) en respectant les critères PRISMA, sur les bases de données PubMed et PsyclNFO. Une étude quantitative a ensuite été réalisée avec 1 325 participants via un questionnaire en ligne, après quoi une étude qualitative avec 39 participants a été conduite. Les entretiens qualitatifs ont été analysés avec la théorie ancrée. Résultats Les patients rapportaient fréquemment des douleurs abdominales, céphalées, nausées/vomissements et des douleurs musculosquelettiques, expliqués par l'anxiété. Développer davantage de somatisations était significativement associé aux événements de vie négatifs (harcèlement, violences physiques, viol), à la dépression, à l'état de stress post-traumatique, à la phobie sociale ainsi qu'aux troubles des conduites alimentaires. La trajectoire des symptômes somatiques a été découpée en quatre phases progressives, de l'émergence à la disparition en cas de déscolarisation. Le principal traitement par distraction était peu efficace, à l'inverse de la compagnie de personnes empathiques et des méthodes d'autothérapie relaxantes. Discussion : Éduquer les patients sur le mécanisme d'installation de la douleur chronique en lien avec la répression des émotions et événements de vie négatifs pourrait faire partie intégrante du traitement des symptômes somatiques du refus scolaire anxieux, en parallèle de l'apprentissage de thérapies psychocorporelles et de l'expression des émotions. Les réarrangements d'emploi du temps ou une réorientation scolaire peuvent parfois également s'avérer utiles. Conclusions : Le traitement doit être multidisciplinaire via une collaboration étroite entre le personnel scolaire, les médecins et les psychologues/psychiatres. Le schéma biopsychosocial est primordial dans l'évaluation des patients. L'utilisation de thérapies psychocorporelles ainsi que thérapies par la conscience et l'expression des émotions restent à tester sur de plus grandes cohortes contrôlées.

Book excerpt: Dans nos sociétés modernes, les difficultés que peuvent rencontrer au cours de leur scolarité certains adolescents, sont un motif fréquent de consultation en pédopsychiatrie. Nous avons réalisé une étude clinique rétrospective afin de déterminer la fréquence des demandes de consultations suggérant un refus scolaire anxieux, d’évaluer la fiabilité de la méthode de sélection utilisée, de décrire la population des patients présentant un refus scolaire anxieux et de définir des profils cliniques. Pour ce faire, nous avons constitué une liste de 183 patients consultant dans un CMP spécialisé dans l’adolescence de Janvier 2009 à Décembre 2012, et sélectionnés par une grille de situations cliniques. Le diagnostic de refus scolaire anxieux (RSA) a été validé par les psychothérapeutes. Puis nous avons étudié les données cliniques des cas de RSA pour définir des profils cliniques. 10 % des demandes de consultations évoquaient un RSA. Après validation clinique par les psychothérapeutes, 83 cas de RSA ont été recensés (4,5% des consultations). La méthode de sélection a permis de repérer 93% des cas de RSA, mais sa valeur prédictive positive n’est que 38%. Elle peut donc être un outil intéressant dans le repérage des cas de RSA. Parmi les patients présentant un RSA, on retrouve autant de garçons que de filles, avec une moyenne d’âge autour de 15,3 ans. 90% d’entre eux présentaient un trouble anxieux, 85% des manifestations somatiques associées. 83% d’entre eux avaient des antécédents de troubles anxieux ou du sommeil dans l’enfance, et on retrouve un trouble anxieux ou dépressif chez 75% des mères. L’anxiété de séparation touche préférentiellement les filles (57% contre 19,5% des garçons). Enfin, nous avons pu identifier trois profils cliniques de patients sujets au refus scolaire anxieux. L’hétérogénéité des tableaux cliniques et l’absence de symptôme spécifique compliquent le repérage des situations de refus scolaire anxieux, dont la prise en charge précoce est un facteur pronostique majeur.
