Book excerpt: En France, les réhabilitations prothétiques par implants dentaires font partie de l'arsenal thérapeutique du chirurgien-dentiste pour le remplacement des dents manquantes chez les patients adultes mais sont, généralement, contre-indiquées chez les enfants et les adolescents. Plusieurs études ont été menées sur la pose d'implants dentaires chez les enfants et les adolescents. Leurs indications et leur impact sur la croissance crânio-faciale suscitent certaines controverses quant à leur utilisation chez ces patients. Ce travail a pour objectif de proposer une revue de la littérature scientifique actuelle sur l'utilisation des implants dentaires, comme alternative aux réhabilitations prothétiques conventionnelles, chez les individus en croissance. Dans la littérature, les recommandations concernant la pose d'implants dentaires chez les individus sains en croissance, préconisent d'attendre la fin de la croissance squelettique et dentaire pour leur utilisation. Un âge arbitraire de minimum 15 ans chez les filles et 18 ans chez les garçons a été déterminé, cependant certains auteurs recommandent même une implantation plus tardive en raison des variations individuelles dans les modèles de croissance. La seule exception à cette règle réside chez les patients en croissance présentant des oligodonties sévères, retrouvées généralement dans les syndromes tels que la dysplasie ectodermique, chez qui la pose d'implants dentaires est autorisée avant la fin de la croissance afin d'améliorer le système stomatognathique dans son fonctionnement et son esthétisme. Chez les patients atteints de dysplasie ectodermique, la National Foundation for Ectodermal Dysplasia (NFED) préconise la mise en place d'implants dentaires comme support de prothèse seulement dans la partie antérieure de la mandibule chez les enfants dès l'âge de 6 ans quand la suture symphysaire est fermée. La pose d'implants dentaires dans un maxillaire en croissance n'est pas recommandée pour la plupart des auteurs en raison du risque d'enfouissement de l'implant dentaire. La pose d'implants dentaires chez les patients en croissance reste une question controversée qui nécessite un diagnostic et un plan de traitement minutieux. La dentisterie restauratrice actuelle se veut être de plus en plus esthétique. Les restaurations postérieures en composite sont aujourd’hui une alternative à l’amalgame. La qualité des matériaux et des systèmes de collage est sans cesse améliorée. Pour autant, ces techniques nécessitent une grande rigueur opératoire et doivent être réservées aux patients dont l’hygiène bucco-dentaire est bonne. Pour les cavités plus larges et étendues, le recours à des méthodes indirectes est recommandé. Il s’agit des inlays, des onlays ou des overlays. En céramique ou en composite, ces restaurations répondent aux impératifs biologiques, mécaniques et esthétiques de la dentisterie actuelle. La conception et la fabrication assistée par ordinateur a fait ses preuves et semble être une technique d’avenir.
