Book excerpt: La douleur est un motif d'inquiéude, et l'un des inconvénients que les patients souhaitent voir réduits au minimum après une chirurgie oral. L'infection, l'anxiété et l'inflammation contribuent à accentuer les difficultés rencontrées par le chirurgien-dentiste lors de l'exercice de sa profession. Les stratégies préventives et curatives en odontostomatologie évoluant continuellement, il est important que les chirurgiens prennent en compte les nouveaux concepts eu égard aux récentes recommandations de l'ANSM et de l'HAS. Certains médicaments couramment utilisés ont été retirés du marché en 2011. Selon les nouvelles recommandations de l'ANSM et de la HAS, une mise à jour de la notion de "patient à risque" doit aider le chirurgien-dentiste à rédiger une prescription d'antalgiquesou d'antibiotiques plus raisonnée. Le praticien est plus souvent amené à rédiger une prescription d'antibiotiques probabilistes ; la prescription documentée est plus exceptionnelle. Un traitement antalgique peut-y être associé mais sera plus efficace et utile en post-opératoire. Une sédation consciente au mélange équimoléculaire oxygène protoxyde d'azote est pratiquée dans les cabinets dentaires depuis 2010 sous certaines conditions. Certains sédatifs n'ont pas d'AMM (Autorisation de Mise sur le Marché) concernant l'indication de la sédation préopératoire d'une chirurgie orale. Cependant, une médication anxiolytique pré-opératoire est souhaitable chez des patients qui redoutent particulièrement tout acte chirurgical. A un moment où la population est plus attentive à la nature et à la qualité des soins prodigués par les professionnels de santé, il est important de souligner les limites des traitements médicamenteux mais aussi leurs éventuelles évolutions. Nous nous interrogerons sur la façon dont le chirurgien dentiste doit s'adapter aux nouvelles recommandations et repenser ses prescriptions en fonction des nombreux progrès réalisés.
