De nombreux syndromes ont été identifiés, ce qui en fait une maladie à la classification complexe. Les traitements adaptés à l’enfant sont nombreux et présentent des effets indésirables importants. Cette maladie est souvent mal vécue par l’enfant et sa famille, car les signes cliniques, les conséquences de la maladie, le traitement et les nombreux préjugés sur cette maladie sont parfois difficile à supporter. La prise en charge de l’épilepsie est primordiale pour sortir de ces préjugés et surtout pour que l’enfant et sa famille mènent une vie ordinaire. Cette thèse a pour but de rappeler les différents syndromes épileptiques et leurs traitements antiépileptiques, ainsi que de montrer l’importance des conseils à donner dans cette maladie.

La crise épileptique est la conséquence d'une décharge neuronale excessive et synchrone anormale. D'étiologies diverses, elle se manifeste différemment en fonction de la localisation du foyer épileptogène. L'enregistrement éléctroencéphalographique est l'examen permettant le diagnostic d'une épilepsie en mettant en évidence des anomalies paroxystiques. Des antiépileptiques adaptés à l'enfant existent. Les plus récents sont mieux tolérés et génèrent moins d'interactions médicamenteuses. La survenue d'une épilepsie peut induire des troubles cognitifs et comportementaux de sévérité variable. C'est le cas des encéphalopathies épileptiques comme le syndrome de West caractérisé par une hypsarythmie, des spasmes infantiles et un retard de développement. Dans la première partie de cette thèse nous aborderons les généralités sur l'épilepsie afin de bien comprendre toutes les facettes de la pathologie, de connaître toutes les techniques de diagnostic ainsi que les traitements disponibles sur le marché et d'évaluer son impact sur la vie quotidienne. Dans le but de conférer un aspect plus concret à toutes ces données, nous nous attarderons sur quelques cas de syndromes épileptiques. Dans un deuxième temps nous nous focaliserons sur le syndrome de West en étudiant également son profil épidémiologique, clinique, thérapeutique, évolutif et pronostique.

Ce type d'épilepsie représente environ 30% des cas traités en France. La neurochirurgie est l'une des techniques la plus efficace dans le traitement de ces crises. Il existe d'autres alternatives thérapeutiques comme la diète cétogène et la corticothérapie. En dehors de la prise en charge thérapeutique de cette pathologie, une adaptation du mode de vie, un accompagnement et l'observance strict du traitement permettront au patient et à ses proches de mener une vie normale
