Book excerpt: Les PASS (Programmes d'Accès aux Soins de Santé) ont vu le jour en France il y a un peu plus de vingt ans afin de lutter contre l'exclusion sociale et ainsi permettre l'accès aux soins des personnes les plus démunies. Leur mission tant médicale que sociale, vise dans un premier temps à accompagner les personnes précaires à accéder au réseau de soins pour qu'elles puissent bénéficier d'une prise en charge, mais également les aider à réaliser toutes les démarches administratives nécessaires à l'obtention du droit commun de la sécurité sociale. Il serait intéressant d'étudier l'organisation d'un tel système avec l'exemple de la PASS dentaire à Strasbourg dans le but de comprendre tous les enjeux dont il est question pour que l'ensemble des usagers concernés puisse être reconduit dans un circuit de soins général. Dans un contexte social où la précarité ne cesse de croitre et où les démarches administratives pour disposer de la sécurité sociale ne cessent d'être plus complexes, le travail de la PASS est décisif. Aussi pour tous les professionnels de santé, il s'agit d'en prendre conscience et de ne pas omettre de leur prise en charge cette population, souvent qualifié « des oubliés » pour laquelle l'accès aux soins considéré par le Code de la Santé Publique comme un droit universel, est néanmoins discutable.

Elle concerne différentes catégories de populations et touche plusieurs domaines (ressources, culture, état de santé... et accès et aux soins). Le bassin rennais n'est pas épargné et présente une proportion importante de personnes en difficulté sociale. Ces dernières ont un besoin en soins bucco-dentaires non négligeable mais ne trouvent pas de structures adaptées à leurs besoins et modes de vie particuliers. En effet, différentes problématiques en lien avec la précarité et l'exclusion sociale peuvent être mises en évidence. En se basant sur ces dernières et en utilisant la méthodologie du cadre logique, nous en sommes arrivés à l'idée de proposer un projet de mise en place d'une structure de soins dentaires, au sein du CHU de Pontchaillou, pour une prise en charge spécifique (à la fois dentaire, sociale et psychologique) de ces patients , dans le cadre des Permanences d'Accès aux Soins de Santé créées par le Programme Régional d'Accès à la Prévention et aux Soins institué par la loi du 29 juillet 1998.

Le praticien est guidé par son éthique propre, par la réglementation déontologique de sa profession mais également par les impératifs administratifs et financiers incombant au chirurgien-dentiste dans la gestion de son cabinet qui reste une petite entreprise.
