Book excerpt: Les antivitamines K (AVK) ont longtemps été la seule classe pharmacologique d'anticoagulant oral. Depuis 2008, il existe une alternative thérapeutique : les anticoagulants oraux directs (AOD) dont les indications ne cessent de s'élargir au cours des années. L'éducation thérapeutique est un élément clé pour la prise en charge des malades sous anticoagulants oraux du fait de leur haut risque iatrogénique. L'objectif de cette étude est de comparer les connaissances des patients sous anticoagulants oraux au long cours selon leur traitement : AVK versus AOD. Les objectifs secondaires sont d'évaluer les modalités de l'information thérapeutique, le ressenti et l'observance des patients selon la classe pharmacologique : AVK ou AOD. Méthodes : C'est une étude épidémiologique descriptive réalisée auprès des patients avec l'aide des médecins généralistes de Seine Maritime, des angiologues de la clinique Mathilde à Rouen, des médecins du service de médecine interne au Centre Hospitalier Universitaire (CHU) de Rouen, entre le 1er mars et le 31 mai 2017. Chaque patient était interrogé par unquestionnaire de 8 questions. Le critère de jugement principal était l'analyse comparative des moyennes des notes obtenues sur la connaissance du traitement entre les patients sous AVK et ceux sous AOD. Résultats : Cent vingt sept patients ont été inclus : 72 dans le groupe AVK et 52 dans le groupe AOD. La connaissance thérapeutique globale était satisfaisante avec une note moyenne respectivement de 5,69/8 dans le groupe AVK contre 5,48/8 dans groupe AOD sans différence significative. La surveillance biologique était significativement mieux connue dans le groupe AVK. Les facteurs influençant l'activité anticoagulante n'étaient pas suffisamment connus dans les deux groupes (régime alimentaire, interactions médicamenteuses). Les patients ayant reçu un livret d'information avaient significativement une meilleure connaissance du traitement anticoagulant que ceux ne l'ayant pas reçu (6,00/8 versus 5,26/ 8). Les patients sous AOD trouvaient le traitement significativement plus satisfaisant que ceux du groupe AVK. Conclusion : Notre étude suggère que les patients connaissent insuffisamment les facteurs influençant l'activité anticoagulante du traitement anticoagulant oral quelle qu'en soit la classe thérapeutique. Au vu du risque iatrogénique clairement identifié et insuffisamment connu, de nouvelles mesures d'éducation thérapeutique plus homogènes pourraient être proposées.
