Book excerpt: OBJECTIFS : L'agitation est un motif de consultation fréquent dans les services d'urgence faisant l'objet de prises en charge variées en l'absence de recommandations actualisées. Notre étude a pour but de comparer la prise en charge du patient agité aux urgences du CHU de Dijon à ces recommandations officielles données par la 9ème Conférence de consensus. MATERIEL ET METHODES : Etude épidémiologique descriptive, rétrospective, mono-centrique conduite dans le service des urgences du CHU de Dijon. La population a été sélectionnée parmi les patients présentant un état d'agitation ou développant une agitation au cours de leur prise en charge, entre le 1er janvier et le 31 décembre 2017. Leur prise en charge a été évaluée comparativement aux recommandations selon 5 points : la réalisation des explorations immédiates, l'abord relationnel, le traitement médicamenteux, la contention et la surveillance. RESULTATS : 192 patients ont été inclus, majoritairement des hommes jeunes dont on connaît peu les antécédents et le traitement lors de l'arrivée. Les étiologies de l'agitation sont essentiellement psychiatriques (50%), organiques (26%) et alcoolique (24%). 76% des patients bénéficient des explorations recommandées, 71% d'une tentative d'apaisement verbal et 73% du traitement médicamenteux adapté à leur cause d'agitation. La contention est prescrite sans sédation associée dans 16% des cas, la fiche de surveillance n'est pas disponible. La surveillance n'est conforme aux recommandations que dans 30% des cas. CONCLUSION : Certaines pratiques peuvent et doivent être modifiées au vu des résultats de cette étude. L'utilisation de fiches dédiées à l'évaluation de l'agitation, à la surveillance ainsi qu'une sensibilisation des soignants à la prise en charge du patient agité devraient être développées tout comme la création d'un protocole décisionnel clair de prise en charge.

Book excerpt: Introduction : L'activité des services d'urgences a augmenté de 30 % en 10 ans avec de nombreuses consultations relevant des soins premiers. Les motivations des patients à consulter aux urgences semblaient mal établies. Méthode : Etude qualitative réalisée au aux urgences du CHU de DIJON, par entretiens semi-dirigés, avec échantillonnage en variation maximale. L'analyse a été effectuée selon le principe de la théorisation ancrée, en double aveugle afin de trianguler des données. Résultats : Vingt entretiens ont été effectués. Les patients avaient des représentations de leur problème de santé souvent éloignées de la réalité anatomo-physiologique et présumaient que les urgences leur apporteraient une réponse claire. La douleur, la crainte d'une pathologie grave et le sentiment de singularité justifiaient leur besoin d'examens complémentaires qu'ils estimaient facilement disponibles aux urgences. Ils avaient également des difficultés à anticiper les rendez-vous en ambulatoire et à naviguer dans le système de soins. La confiance accordée au médecin généraliste et à son rôle dans la prise en charge des soins non-programmés était très variable. Conclusion : Les patients souhaitaient une réponse médicale sure, rapide et adaptée à une véritable urgence ressentie. Ils choisissaient les services d'urgence du fait de leur accessibilité, du plateau technique et d'une insatisfaction ou incompréhension du système de soins. Le médecin traitant garde un rôle important et pourrait influencer les patients dans leur choix de recours ou non aux urgences grâce à une éducation qui pourrait se faire via l'accompagnement dans leurs recherches internet. Une amélioration de l'offre de soins non programmés pourrait également être intéressante avec des cabinets sans rendez-vous munis d'un plateau technique minime.

Dans l’optique d’une réduction des temps d’attente et de passage des patients, nous avons élaboré une réorganisation du circuit court des urgences du CHU de Dijon via la mise en place d’un circuit ultra court. Un protocole de coopération a été conçu pour que les attelles plâtrées et les pansements soient réalisés, après formation, par un Aide Soignant Diplômé d’Etat à la place d’un médecin. Nous avons évalué ce procédé par une étude comparative rétrospective entre 2012 et 2013. Les résultats montrent une amélioration des temps d’attente globaux de 12 minutes et une réduction des temps de passage de 60 minutes au maximum, tout diagnostic confondu. Cette nouvelle organisation de travail est appréciée des patients qui la considèrent comme excellente pour 42% d’entre eux. Par l’introduction d’un transfert de compétences entre médecin et aides soignants, nous démontrons l’amélioration du service rendu aux patients. La diminution du temps de prise en charge des malades et la valorisation d’une profession paramédicale sont les effets bénéfiques de notre réorganisation. La modification de ces pratiques professionnelles s’inscrit dans l’optimisation des soins dispensés aux urgences.

Une étude rétrospective sur 3 années successives, entourant l’année d’ouverture de la structure autorisée à l’urgence au sein de la clinique de Chenôve a été réalisée pour vérifier l’impact de celle-ci sur l’activité des urgences du CHU de Dijon. Il en résulte que l’ouverture de cette nouvelle structure, malgré plus de 10 000 passages par an, ne diminue pas l’activité du SAU mais vient officialiser une activité d’urgence des cliniques qui existait déjà tout en créant une nouvelle offre de soins, particulièrement dans le domaine de la petite traumatologie et des malades classés CCMU-CEMSA 1 et 2. Tout ceci est renforcé par une offre de soins de ville structurée dans l’agglomération dijonnaise, ce qui limite au SAU de Dijon les patients relevant de la médecine générale qui ne représentent que 10 % des passages.

L’étude Archiflux réalisée dans le cadre de ce travail est centrée sur le parcours patient au sein des urgences avec comme objectif de dresser un état des lieux des flux de patients afin d’adapter l’architecture et l’organisation du service régionale d’accueil des urgences du Centre Hospitalier Universitaire de Dijon aux besoins du flux actuel et futur. Ce projet, basé sur une approche processus et s’inscrivant dans le cadre de l’amélioration continue de la qualité, aura permis de recueillir plus de 702 formulaires patient et de traiter les résultats pour mettre en avant les axes d’améliorations de la prise en charge du patient en s’appuyant sur les indicateurs de la littérature et des recommandations de bonnes pratiques. Les recommandations préconisées dans ce travail permettront d’enrichir la réflexion et de suggérer de nouvelles actions concrètes permettant à terme, d’optimiser la gestion du flux de patients aux urgences et de parvenir à une amélioration notable de la qualité et de la sécurité de la prise en charge du patient de façon globale.

Growing interconnectedness of global economies facilitates the spread of the effects of the financial crises. Financial crises cause severe difficulties for persons to fulfill their contractual obligations. During the financial crises, performance of contractual obligations may become excessively onerous or may cause an excessive loss for one of the contracting parties and consequently destroy the contractual equilibrium and legitimate the governmental interventions. Uncomfortable economic climate leads to one of the most controversial dilemmas of the contract law: whether the binding force of the contract is absolute or not. In other words, unstable economic circumstances impose the need to devote special attention to review and perhaps to narrow the binding nature of a contract. Principle of good faith and fair dealing motivate a variety of theoretical bases in order to overcome the legal consequences of financial crises. In this book, all these theoretical bases are analyzed with special focus on the available remedies, namely renegotiation, rescission or revision and the circumstances which enables the revocation of these remedies. The book collects the 19 national reports and the general report originally presented in the session regarding the Effects of Financial Crises on the Binding Force of Contracts: Renegotiation, Rescission or Revision during the XIXth congress of the International Academy of Comparative Law, held in Vienna, July 2014.

The Environmental Protection Agency's decision in the summer of 2001 to delay implementing a new, more stringent standard for the maximum allowable level for arsenic in drinking water generated a great deal of criticism and controversy. Ultimately at issue were newer data on arsenic beyond those that had been examined in a 1999 National Research Council report. EPA asked the National Research Council for an evaluation of the new data available. The committee's analyses and conclusions are presented in Arsenic in Drinking Water: 2001 Update. New epidemiological studies are critically evaluated, as are new experimental data that provide information on how and at what level arsenic in drinking water can lead to cancer. The report's findings are consistent with those of the 1999 report that found high risks of cancer at the previous federal standard of 50 parts per billion. In fact, the new report concludes that men and women who consume water containing 3 parts per billion of arsenic daily have about a 1 in 1,000 increased risk of developing bladder or lung cancer during their lifetime.
