Avec la récente revalorisation tarifaire, le développement des formations continues et la possible coopération entre les acteurs médicaux, les sages-femmes devraient s'approprier ces missions de santé publique.

Book excerpt: Objectif : Définir si en 2014 les sages-femmes libérales de Savoie et Haute-Savoie pratiquent des consultations de suivi gynécologique de prévention ou des consultations de contraception. Définir les caractéristiques professionnelles influençant la pratique de ces activités, les formations continues et le souhait de développer ces activités. Matériel et Méthode : Il s'agit d'une étude descriptive réalisée auprès de 107 sages-femmes libérales des départements de Savoie et Haute-Savoie. Un questionnaire disponible en ligne leur a été envoyé. Le critère de jugement principal est le pourcentage de sages-femmes faisant appel aux nouvelles compétences au sein de leur activité professionnelle. Les critères de jugements secondaires sont les caractéristiques professionnelles, les formations continues, le souhait des sages-femmes de développer une activité gynécologique ou contraceptive. Résultats : Soixante-quatre sages-femmes libérales ont participé à l'enquête et 78% d'entre-elles font appel aux nouvelles compétences au sein de leur activité professionnelle. Seule l'année d'obtention du diplôme est une caractéristique influençant la pratique de consultations de contraception et de gynécologie (p = 0,009). La formation initiale n'a pas d'influence (p = 0,6) : le souhait de pratiquer ces activités ou la qualité de la formation initiale pouvant être les principales causes. Les sages-femmes souhaitant développer une activité de contraception devront mettre en place une formation continue. Conclusion : Les sages-femmes ont développé une activité de gynécologie et de contraception au sein de leur activité professionnelle. Les sages-femmes diplômées après 2009 sont plus susceptibles de faire appel aux nouvelles compétences.

Book excerpt: Objectif de l'étude : Notre étude proposait d'établir un état des lieux de l'offre en médecine préventive proposée par les sages-femmes libérales dans la région de Franche Comté. Nous souhaitions évaluer la réalité de l'exercice des sages-femmes libérales dans le domaine de la contraception et du suivi gynécologique de prévention dans notre région depuis l'acquisition de nouvelles compétences. Matériel et méthodes : Etude descriptive prospective menée auprès de 52 sages-femmes libérales de Franche-Comté. Le recueil des données a été effectué à partir d'un questionnaire. Résultats : L'étude a montré que 69,2 % des sages-femmes libérales ont investi pleinement leurs nouvelles compétences en proposant des consultations gynécologiques de prévention à leurs patientes. Elles ont toutes bénéficié d'une formation en amont pour actualiser leurs connaissances en gynécologie et en contraception. Les sages-femmes libérales évoquent encore des difficultés face à la mise en application de leurs nouvelles compétences. Elles aimeraient qu'un réseau de correspondance se développe davantage dans la région pour les soutenir. Concernant les sages-femmes libérales qui ne pratiquent pas de consultations de médecine préventive, 67,5 % ont pour objectif de se former dans les années à venir. Conclusion : Le manque de reconnaissance par les professionnels de santé et la méconnaissance de la population sur le champ d'activité des sages-femmes constituent une limite majeure à leur action de prévention. Les sages-femmes libérales doivent affirmer leur autonomie au sein de cette profession pour se placer comme professionnels de premiers recours dans la santé des femmes.

Book excerpt: Cette étude a pour objectif d'identifier la population de sages-femmes libérales des Pays-de-la-Loire qui pratiquent le suivi gynécologique préventif ou contraceptif, puis de décrire cette activité et les formations reçues. Par un questionnaire adressé aux 278 sages-femmes libérales de la région, nous avons obtenu un taux de réponse de 71%. 57.6% d'entre elles pratiquent des consultations de gynécologie ou de contraception. Elles effectuent le plus souvent des actes classiques de dépistage par FCV ou de prescription de contraceptifs, et d'autres ont une activité plus complète. 87.7% d'entre elles ont pour cela suivi une formation complémentaire. Il est donc nécessaire d'adapter la formation initiale à cette pratique. L'offre de soins en gynécologie et en contraception est en plein essor. Elle ne pourra toutefois être optimale que par un travail en réseau entre les différents professionnels. Les femmes pourront ainsi bénéficier rapidement du suivi approprié.

L'étude réalisée pour ce mémoire avait pour but d'évaluer la pratique de ces activités dans la région Basse-Normandie, grâce à un questionnaire en ligne, distribué aux sages-femmes libérales, territoriales et hospitalières réalisant des consultations programmées. Elle nous montre que les sages-femmes ont pris possession de ces nouvelles compétences puisque 57% d'entre-elles les pratiquent, majoritairement dans les secteurs libéraux et territoriaux. La pratique du suivi gynécologique de prévention et des consultations de contraception par les sages-femmes est destinée à continuer son ascension : plus de 60% des sages-femmes qui ne les pratiquent pas sont intéressées par ces activités ; les sages-femmes nouvellement diplômées vont avoir bénéficié de la dernière formation initiale de 2011 renforcée en pratique clinique en gynécologie et la loi conforte leur rôle en tant que professionnelles de premier recours en leur donnant maintenant accès à la réalisation des interruptions volontaires de grossesse médicamenteuses.

Characterized by a very dense visual style, featuring flowery backgrounds and big-eyed, androgynous boys and girls, it is an extremely popular and prominent genre in Japan. Why is this genre so appealing? Where did it come from? Why do so many of the stories feature androgynous characters and homosexual romance? Passionate Friendship answers these questions by reviewing Japanese girls’ print culture from its origins in 1920s and 1930s girls’ literary magazines to the 1970s “revolution” shojo manga, when young women artists took over the genre. It looks at the narrative and aesthetic features of girls’ literature and illustration across the twentieth century, both pre- and postwar, and discusses how these texts addressed and formed a reading community of girls, even as they were informed by competing political and social ideologies. The author traces the development of girls’ culture in pre–World War II magazines and links it to postwar teenage girls’ comics and popular culture. Within this culture, as private and cloistered as the schools most readers attended, a discourse of girlhood arose that avoided heterosexual romance in favor of “S relationships,” passionate friendships between girls. This preference for homogeneity is echoed in the postwar genre of boys’ love manga written for girls. Both prewar S relationships and postwar boys’ love stories gave girls a protected space to develop and explore their identities and sexuality apart from the pressures of a patriarchal society. Shojo manga offered to a reading community of girls a place to share the difficulties of adolescence as well as an alternative to the image of girls purveyed by the media to boys and men. Passionate Friendship’s close literary and visual analysis of modern Japanese girls’ culture will appeal to a wide range of readers, including scholars and students of Japanese studies, gender studies, and popular culture.

Frank Porporino and I arrived at the starting point for our work together by very different routes. Originally trained as an experimental psychologist, I had become in creasingly restive within the confines of the laboratory, and spent a sab batical year in the equivalent of a clinical internship. I then spent some time as a part-time consultant in a local penitentiary. Most of my time in the institution was spent with inmates with a variety of problems, probably about 50 individuals over the course of a year. Although this was far fewer than a full-time psychologist in the system might encounter, it served as a quick cram course on problem prisoners and prisoner problems. Very quickly my stereotypes about convicts were shown to be virtually useless. I learned that the criminal classes included all levels of society, and that the behavior of prisoners was the same as that of other human beings in a difficult environment.
