Book excerpt: L'implantologie est aujourd'hui une discipline incontournable pour le praticien, ses indications ne cessent de s'étendre. Pour y accéder, l'imagerie médicale est un support indispensable tant sur le plan médico-légal que d'aide au diagnostic. Grâce aux progrès techniques, les possibilités de visualisation des structures se sont multipliées. C'est pourquoi, le praticien doit effectuer des demandes raisonnées et savoir exploiter ses examens. En première intention, la radiologie conventionnelle reste indispensable mais elle montre ses limites notamment pour explorer les structures en trois dimensions. Le scanner s'est imposé comme une technique d'imagerie répondant à ses exigences. Il reste alors à transposer les connaissances apportées par cette imagerie à la réalisation clinique des actes thérapeutiques. Ce sont tout d'abord les maquettes tri-dimensionnelles qui ont permis de réaliser une simulation pré-chirurgicale, puis les logiciels de simulation. Ils offrent au praticien, la possibilité de simuler sur son écran d'ordinateur son projet implantaire et de visualiser le site en trois dimensions. Puis, pour transférer en bouche les données informatisées du projet implantaire, des guides chirurgicaux spécifiques ont été mis au point. Nous entrons ainsi dans l'ère nouvelle de la chirurgie assistée par ordinateur ouvrant le champ d'action au domaine de la chirurgie navigationnelle.

L'imagerie médicale occupe une place majeure dans ce processus. En France, chaque année 3,6millions de clichés panoramiques et plus de 11 millions de radiographies intra-orales sont réalisés en moyenne dans les cabinets dentaires hospitaliers ou de ville. Ces données sont mobilisables lors de la collecte d'informations préalables à la confrontation des données ante et post-mortem lors des étapes d'identification de victimes isolées ou de masse. Il est relativement aisé d'établir une correspondance entre ces clichés grâce à la présence d'éléments exogènes (restaurations, prothèses, corps étrangers), mais la prévalence des lésions carieuses et, a fortiori, de ces éléments exogènes allant diminuant grâce aux nombreuses mesures de prévention, le défi actuel est de réussir à identifier ou cerner des profils grâce aux éléments anatomiques propres aux individus. Avec l'essor de l'imagerie tridimensionnelle, le cadre de vue classique du petit cliché intra-oral que nous connaissons tous s'agrandit ainsi avec une multiplication des informations anatomiques osseuses, dentaires, leurs particularités, des marqueurs de soins, des séquelles d'accidents formant une archive individuelle peu exploitée en soins courants mais précieuse lors d'une identification odontologique. Cette thèse a eu pour but de décrire l'état actuel des techniques d'imagerie en odontologie médico-légale, ainsi que d'évaluer si les techniques utilisées avec l'imagerie 2D pouvaient s'appliquer à l'imagerie 3D, notamment la technique d'évaluation de l'âge à partir des volumes pulpaires, décrite par Kvaal et al. en 1995.

Les techniques d'imagerie, qu'elles soient intra-orales ou extra-orales, bidimensionnelles comme tridimensionnelles, de première comme de seconde intention, ont des applications en identification odontologique post-mortem afin de rechercher des indices odonto-stomatologiques radiologiques dans le but d'orienter l'investigateur vers une identification qu'elle soit positive, probable, exclue ou à défaut vers la constitution d'un profil biologique du défunt. Il existe une mosaïque de techniques pour une grande variabilité de situations dont le choix se fera au cas par cas, par la prise en compte de paramètres cliniques, logistiques et techniques, motivés par les premières investigations et observations cliniques réalisées par l'odontologiste et la composition d'un dossier dentaire ante-mortem. L'essor de nouvelles technologies comme l'imagerie 3D et l'intelligence artificielle font que les techniques d'identification sont vouées à s'améliorer, à gagner en qualité, en précision et en rapidité.
