Book excerpt: Les mini-vis orthodontiques procurent un renfort d'ancrage appréciable. Toutefois, la part d'échecs n'est pas négligeable. L'étude clinique rétrospective réalisée à la Faculté de Chirurgie dentaire de Paris 7, sur les trois dernières années, fait état d'un taux d'échec de24.2% (23 sur 95) ; soit près du double de celui rapporté dans la littérature. La majorité des échecs sont liés à une perte de stabilité primaire. Ils sont survenus pour plus d'un tiers d'entre eux avant mise en charge orthodontique. Un certain nombre d'hypothèses sont proposées pour tenter d'expliquer ce résultat pour le moins surprenant. La maîtrise des facteurs de risque, une meilleure sélection des indications, ainsi qu'une formation des étudiants axée sur la pose ses mini-vis contribueraient vraisemblablement à une augmentation du taux de succès. Mais il ne faut pas oublier que les mini-vis ne sont que des outils au service de l'orthodontie ; et que le plus important, c'est d'aboutir, in fine, au succès du traitement orthodontique, dans l'intérêt du patient.

L’implantologie permet l’obtention d’un ancrage squelettique absolu indépendamment de la collaboration du patient. Les recherches dans ce domaine ont conduit à la mise au point de différents systèmes allant de l’implant dentaire aux mini-vis; en passant par les implants palatins à usage orthodontique et les mini-plaques. Grâce à la réduction de la taille des implants, les mini-vis permettent d’augmenter le nombre de sites d’ancrage. Leur utilisation est très simple, et leur mise en charge est immédiate. Cependant, il existe des risques d’endommager les structures voisines lors de leur insertion, comme par exemple les racines dentaires. Leurs indications sont nombreuses, mais, l’ancrage par les mini-vis ne doit pas systématiquement se substituer aux ancrages conventionnels il doit permettre de simplifier des situations d’ancrages difficiles ou d’apporter de nouvelles options thérapeutiques, en privilégiant par exemple des traitements sans extraction ou sans chirurgie orthognathique. L’utilisation d’un ancrage squelettique trouvera à l’avenir de plus en plus d’applications en pratique orthodontique. Mais, des recherches futures impliquant des essais randomisés cliniques sont nécessaires pour évaluer les facteurs des taux de succès des mini-vis et ainsi rendre leur utilisation la plus sûre possible, car de telles études font actuellement défaut.

Selon la littérature, il est plurifactoriel impliquant l'hôte, le site, la mini-vis, le protocole chirurgical, la procédure orthodontique et la maintenance. Parmi les nombreux facteurs incriminés, les niveaux de preuve les plus élevés désignent inflammation, site mandibulaire, âge jeune, épaisseur corticale fine. Notre étude clinique rétrospective sur 5 ans porte sur 379 mini-vis pour 147 patients dans le cadre du service d'Odontologie de Rangueil. Le taux global d'échec est de 15%. Des facteurs de risque significatifs sont relevés tels qu'une hygiène médiocre, la localisation postérieure, le site mandibulaire et la repose extemporanée pour stabilité primaire insuffisante. Des tendances (ns) apparaissent pointant la spécialité de l'opérateur, le côté droit et le diamètre. Des pistes de recherche sur la nature du bilan radiologique et une extraction proximale à la vis, explorées pour la première fois, sont à creuser.
