Cette branche de l'odontologie ne doit cependant pas être exclusive au parodontologiste et doit intervenir dans le diagnostic, le pronostic et le plan de traitement de tout chirurgien-dentiste. En effet, une prise en compte de l'état de santé parodontale est fondamentale lorsque la décision de conservation ou d'extraction dentaire doit être prise. Cette évaluation se base sur la connaissance des facteurs généraux du patient et doit tenir compte de l'état de santé ainsi que de la motivation de ce dernier. Enfin, les facteurs locaux doivent également être étudiés : l'hygiène bucco-dentaire, l'anatomie dentaire, la perte d'attache, la présence de poche parodontale, le saignement au brossage, le taux d'alvéolyse, l'atteinte furcatoire, la mobilité et la prise en considération d'un implant et d'une couronne implanto-portée à la place d'une dent parodontalement atteinte, sont autant d'éléments importants dans la décision finale. Le but de ce travail est de déterminer jusqu'à quel point il est possible de conserver une dent. En tenant compte des facteurs cités préalablement et en connaissant la probabilité de réussite d'un traitement parodontal sur plusieurs années, il est possible de sauver un nombre plus important de dents. En effet, il est intéressant de constater que même parodontalement affaiblies, beaucoup de dents peuvent être conservées et pour de multiples années.
