Book excerpt: L’hospitalisation à domicile (HAD) vise à réduire ou éviter une hospitalisation, elle pourrait être mise en place à partir d’un service d’Urgence (SU). Une première partie de ce travail rapporte une confrontation de la littérature aux modes de prise en charge décrit par le guide méthodologique du recueil standardisé des données. L’objectif de la seconde partie était de décrire les demandes d’HAD faites depuis un SU. Entre le 1 janvier 2014 et le 31 mars 2015 toutes les demandes faites auprès de l’HAD de Crest depuis un SU ont été analysées selon quatre axes : les caractéristiques sociodémographies, l’environnement et conditionnement du patient avant son admission au SU, les conditions d’admission et prise en charge au SU et enfin les pour les demandes ayant été acceptées les caractéristiques de la prise en charge HAD. 42 demandes ont été réalisées; 13 ont été refusées et 29 acceptées. 26 patients vivaient à leur propre domicile. Un aidant au même domicile était retrouvé dans 33 cas (78.6%). Le motif d’admission le plus retrouvé était la dyspnée [15 cas (35.7%)]. Il a été retrouvé 24 cas (57.1%) de pathologies infectieuses dont 17 cas (40.5%) de pneumopathies. Parmi les demandes acceptées le motif de prise en charge principal le plus retrouvé était ‘’ soins palliatifs’’ et ‘’traitement intraveineux’’. Ces résultats ont permis l’élaboration d’une fiche d’aide à la demande d’HAD depuis un SU. Bien que manquant de puissance cette étude est innovante. Notre étude replace l’HAD comme acteur des soins palliatifs. En accord avec les données de la littérature la prise en charge des pneumopathies en HAD à partir d’un SU à toute sa place. Cette étude replace le médecin traitant et les aidants familiaux comme élément essentiel à une prise en charge en HAD. La prise en charge des pneumopathies et de patients relevant de soins palliatifs sont les indications principales pour lesquelles une demande d’HAD peut être faite depuis un SU
