Selon Gilbert et al, 10% des enfants sont victimes de maltraitance dans les pays à hauts revenus. Le corps médical est au premier rang pour le repérage des enfants victimes. Mais comment expliquer que seul 5% des signalements de maltraitance émanent des médecins ? Le but de notre étude était d'analyser le vécu et le ressenti des médecins généralistes face à une suspicion de maltraitance, afin d'identifier les freins potentiels aux signalements. Nous proposerons ensuite d'éventuelles pistes d'améliorations dans la prise en charge des enfants maltraités. Matériels et méthodes : Il s'agit d'une étude qualitative par méthodologie théorique, avec réalisation de neuf entretiens compréhensifs auprès de médecins généralistes d'Ile De France. Ils étaient enregistrés sur dictaphone puis retranscrits intégralement. Le codage des entretiens a permis l'analyse de cinq thèmes : connaissances sur la maltraitance infantile, vécu et ressenti concernant une situation de maltraitance, représentation des certificats médicaux obligatoires, vécu et ressenti concernant les retours des informations préoccupantes et signalements, et la communication entre médecins généralistes et l'hôpital. Résultats : Notre étude montre que la majorité des médecins interrogés estiment manquer de formation et d'information sur la maltraitance infantile. Certains se sentent gênés devant une suspicion de maltraitance et inquiets vis à vis de la réaction parentale et de l'erreur diagnostique. La solitude est évoquée : certains évoquant leur isolement comme un frein, d'autres soulignant la possibilité d'en discuter avec leurs collègues. Conclusion : La communication ville hôpital et interdisciplinaire doit être améliorée. Il semble nécessaire d'insister sur la formation continue et la distribution d'informations afin que les médecins généralistes puissent avoir un meilleur ressenti face à la maltraitance.

Les médecins généralistes suivent 40% des moins de 3 ans, ils ont donc un rôle clé dans le repérage et la prise en charge, cependant seul 5% des signalements proviennent du corps médical. Notre objectif était de connaître les freins au dépistage et à la prise en charge des enfants en danger par les médecins généralistes vendéens. L'objectif secondaire était l'élaboration de pistes d'amélioration permettant la réduction de ces freins. Matériel et méthode : nous avons réalisé une étude épidémiologique prospective, du 15 mai au 1er septembre 2017, à l'aide d'un questionnaire diffusé par voie postale auprès des 459 médecins généralistes vendéens. Il était composé de 31 questions, permettant de connaître le profil des médecins, leurs connaissances sur l'enfant en danger – définition, formation, aspect législatif, recommandation HAS, repères cliniques- leur expérience personnelle, et les freins qu'ils rencontraient. Résultats : nous avons recueilli 100 questionnaires, soit un taux de participation de 21.8%. On observait une difficulté d'appropriation de la définition de l'enfant en danger, avec l'identification de la violence physique par 94% des médecins, mais seule la moitié identifiait la négligence. Les médecins avaient conscience de l'importance de ce fléau, avec 74% l'estimant =10%. Pourtant seuls 41.2% ont été face à un cas avéré de maltraitance au cours de leur carrière, sans différence selon la durée d'exercice ; avec uniquement 50% ayant eu la bonne démarche d'alerte. Ils obtenaient un bon score moyen sur les repères cliniques (73.8/100), avec cependant des facteurs risques non identifiés comme la prématurité (34%), et des à priori persistants comme une fréquence plus important du syndrome du bébé secoué dans les milieux défavorisés (35%). Les freins majeurs à la réalisation d'une IP ou d'un signalement étaient similaires : le manque de connaissance des procédures (respectivement 23.7% et 20.6%), la peur de se tromper (30.9% et 19.6%), l'absence de retour (18.6% et 17.5%), le sentiment d'isolement (18.6%, 15.5%). Conclusion : afin de diminuer les obstacles du repérage et de la prise en charge, il nous semble important d'organiser la mise en place de formations interprofessionnelles pour les internes et les médecins généralistes, de favoriser les échanges entre les acteurs locaux avec des réunions de présentation et le retour systématique d'information, enfin la création d'une brochure départementale d'information avec notamment les coordonnées des acteurs principaux.

Brown’s writing is clear and aims to describe the various theoretical ideas as plainly as possible. Detailed clinical material is given in most chapters to illustrate the theoretical perspectives. Brown applies this theory of transformational processes to a variety of topics, including the analyst’s receptivity, countertransference as transformation, the analytic setting, the paintings of J.M.W. Turner, "autistic transformations" and other clinical situations in the analysis of children and adults. Fred Busch ponders if it has been fully recognized that some post-Bionions suggest a new, radical paradigm for what is curative in psychoanalysis. After searching for the roots of the analyst’s use of reverie in Bion’s work and questioning whether in this regard Bion was a Bionian, Busch carefully examines the work of some post-Bionians and finds both convincing ways to think about the usefulness and limitations of the analyst’s use of reverie. He explores questions including: From what part of the mind does a reverie emerge? How does its provenance inform its transformative possibilities? Do we over-generalize in conceptualizing what is unrepresented, with the corresponding problem of false positives? Do dreams equal understanding and what about the generalizability of the co-created reverie? Busch concludes that it is primarily through the analyst’s own associations that the reverie’s potential is revealed, which further helps the analyst distinguish it from many other possibilities, including the analyst’s countertransference. He believes in the importance of converting reveries into verbal interpretations, a controversial point amongst post-Bionians. Busch ends with the difficult task of classifying the analyst’s reveries based on their degree of representation. The Analyst's Reveries will be of great interest to psychoanalysts and psychoanalytic psychotherapists.
