Les personnes sous traitement continu avec un antagoniste de la vitamine K (AVK) présentent un risque accru de complications hémorragiques pendant et après ce type d'interventions. Le but de cette revue systématique est d'évaluer l'efficacité et la sécurité de l'acide tranexamique (AT) pour réduire le risque de saignement post-opératoire chez des patients sous anticoagulants oraux (AO) nécessitant une chirurgie dento-alvéolaire. Sur 286 articles éligibles, 150 ont été exclus, 21 articles ont été étudiés et 6 ont été sélectionnés en raison de leur pertinence pour répondre à la problématique de notre étude. Il a ainsi pu être mis en évidence l'efficacité et la sécurité de l'utilisation de l'AT chez les patients sous AVK pour prévenir le risque hémorragique après une extraction dentaire. Une solution buvable d'AT à une concentration de 4,8-5% peut être utilisée en utilisation topique (compression et bain de bouche) en vu de prévenir le saignement, et sera prescrit pendant 2 à 7 jours, 3 à 4 fois par jour. Les résultats indiquent que l'AT est cliniquement efficace et sans danger pour les patients traités par des AO recevant une chirurgie dento-alvéolaire.
