La douleur est une expérience individuelle complexe, faisant appel à des sensations, associées à des émotions désagréables. La définition et la quantification de ces deux composantes sont influencées par les caractéristiques de l’individu et son histoire personnelle. Le rôle du praticien est de comprendre l’état de douleur du patient le plus fidèlement, afin de lui procurer un soulagement optimal. C’est pourquoi la consultation d’urgence doit s’attacher à définir l’étiologie des douleurs, mais aussi de comprendre les caractéristiques individuelles de l’expérience. L’évaluation, trop peu employée dans la discipline, représente une étape indispensable puisqu’elle permet de mesurer objectivement l’intensité de la douleur. Une étude a été menée dans l’unité fonctionnelle des urgences du centre Abel Caumartin du CHRU de Lille afin d’apporter des connaissances supplémentaires sur la douleur aiguë dentaire, son intensité et l’influence des paramètres individuels. La conjugaison de connaissances pointues du phénomène douloureux et la prise en compte de l’individualité de chaque patient, apporteront un meilleur soulagement et contribueront à lutter contre l’habitude du patient à percevoir la douleur dentaire comme un fléau.

Des aspects auparavant peu pris en compte sont également exposés, comme la génétique et la génomique fonctionnelle, la pharmacogénétique des analgésiques et le rôle des stéroïdes endogènes. Enfin les classes de médicaments utilisés quotidiennement pour soulager ces différents types de douleurs font également l'objet de chapitres indépendants, AINS, antalgiques opioïdes et non opioïdes, antidépresseurs et antiépileptiques. Sans oublier les données récentes sur les bases neurobiologiques des effets placebo et nocebo qui jouent un rôle fondamental dans la réponse thérapeutique. Cet ouvrage très complet, abordant principalement des données fondamentales mais aussi cliniques, sera très utile aux professionnels de santé ainsi qu'à tous les étudiants du module 6 des études de médecine et des différents diplômes de la douleur (capacité, DESC, DIU) désireux d'approfondir leurs connaissances .
