Book excerpt: L’arthropathie chronique est une complication majeure de l’hémophilie. touchant le plus fréquemment le genou, elle évolue vers une destruction articulaire sévère, entraînant des douleurs et une raideur responsable d’un handicap fonctionnel majeur. La réalisation d’une arthroplastie du genou chez ces patients est considérée comme difficile, entraînant un taux important de complications avec des résultats à long terme variables. Le but de cette étude est d’évaluer les résultats cliniques et fonctionnels de l’arthroplastie totale du genou de première intention à travers notre expérience et les résultats de la littérature. Nous rapportons les résultats de 32 prothèses, réalisées chez 24 patients entre 1987 et 2010 de manière rétrospective et exhaustive avec un recul moyen de 8,2 ans (2 à 23 ans). L’âge moyen des patients lors du geste chirurgical est de 39 ans (20 à 63). Tous étaient des hémophiles sévères, 2 présentaient un inhibiteur circulant, et excepté dans un cas, la contamination virale était constante (11 patients VIH positifs, 3 VHB positifs et 19 VHC positifs). Les complications post-opératoires sont fréquentes (37,5%) : 2 thromboses veineuses, 1 hémorragie aigue, 4 hémarthroses, 2 fractures, 4 infections dont une infection de la prothèse qui conduira à une arthrodèse. On relève 3 reprises chirurgicales pour usure sans descellement. Le gain d’amplitude est modeste (19°), sans modification de la flexion. En revanche le flessum est diminué de manière significative (24,8° à5,5°) permettant de retrouver un secteur fonctionnel utile à la marche. L’amélioration du score IKS genou est modeste (gain de 50,6) avec un score fonction restant médiocre expliqué par l’atteinte polyarticulaire de la maladie. Nous observons un taux de survie de 80% à 10 ans, ce qui reste inférieur à celui d’une population non hémophilique. Malgré cela l’arthroplastie totale du genou permet une amélioration considérable de la douleur et de la qualité de vie chez des patients jeunes et actifs justifiant sa mise en place.

L'objectif de notre étude était de déterminer l'influence des mobilités articulaires pré-opératoires du genou sur les scores fonctionnels post-opératoires des PTG chez les hémophiles. MATERIEL ET METHODES. Une étude rétrospective monocentrique de 25 PTG consécutives chez des patients hémophiles sévères opérés entre 1998 et 2014 a été réalisée. Les critères d'inclusion étaient les patients hémophiles sévères ayant une arthropathie hémophilique traitée par prothèse totale de genou à l'exception des reprises de PTG. Les scores fonctionnels KSS, HSS et WOMAC pré et post-opératoires ont été comparés. L'évolution des mobilités articulaires et l'influence des mobilités articulaires pré-opératoires sur les scores fonctionnels post-opératoires ont été analysées. RESULTATS. Vingt trois PTG chez 18 patients hémophiles ont été incluses. L'âge opératoire moyen était de 44,5 +7 ans. Le recul moyen était de 83,7 +50 mois. La différence entre les scores pré et post-opératoires était significative. L'amplitude articulaire moyenne était de 67,2 +30° en pré-opératoire et de 74,2 +36° au dernier recul (p=0,12).
