La gestion de l’implant et ses complications impliquent principalement quatre longueurs d’ondes : les lasers CO2 (10600 nm), Er :YAG (2940 nm), Nd :YAG (1064 nm) et enfin les lasers Diodes (de 810 à 980 nm). Le choix d’une longueur sera constamment orienté par une réflexion préalable du praticien sur l’effet recherché. Les applications cliniques des traitements « laser-assistés » comprennent la chirurgie pré-prothétique, l’operculisation des implants enfouis, le modelage des tissus mous péri-implantaires, le traitement de la péri-implantite et la désinfection des surfaces implantaires, enfin l’accélération de la cicatrisation osseuse et muqueuse grâce à la biomodulation. En conclusion, la technologie laser peut suppléer les techniques conventionnelles voire les remplacer dans certains cas. Ces traitements « laser-assistés » constituent des perspectives prometteuses en termes d’efficacité, d’amélioration des suites post-opératoires et d’accélération de la cicatrisation.

Chaque laser se caractérise en particulier par son milieu actif, ce qui définit sa longueur d’onde. Le laser a plusieurs effets sur les tissus. C’est l’effet thermique qui est le plus recherché : cela va de l’échauffement à la destruction selon la puissance du rayonnement. Les tissus parodontaux sont riches en eau et en hémoglobine. Les lasers que nous utilisons doivent avoir une longueur d’onde correspondante au pic d’absorption de ces deux composants. Cinq types de laser sont donc principalement utilisés en chirurgie dentaire : diode, CO2, Er:YAG et Nd:YAP. Les lasers en parodontologie sont utilisables pour les surfaçages et le traitement des péri-implantites, en complément des méthodes traditionnelles (ultrasons et curettes manuelles). Ils permettent également la chirurgie de tissus mous le plus souvent sans anesthésie, sans suture, sans saignement et avec une cicatrisation rapide. Ce nouvel outil nécessite encore de nombreuses études pour parfaire les protocoles d’utilisations, protocoles qui doivent être scrupuleusement respectés sous peine d’échec thérapeutique.

Available in PDF, EPUB and Kindle. Book excerpt: À l'heure d'aujourd'hui, notre espérance de vie augmente ainsi que nos attentes envers les progrès de la science médicale. Ainsi depuis plusieurs années les implants sont utilisés dans le traitement des édentements. Plus de 10 millions d'implants sont posés chaque année dans le monde. De nos jours, le taux de survie à 5 ans est passé à 97.1% contre le 93.5% évaluable avant les années 2000. La péri-implantite a été décrite en 2008 lors de la 6ème conférence de consensus européenne en Parodontologie comme une inflammation de la muqueuse associée à une perte osseuse péri-implantaire. Cette maladie infectieuse à étiologies multiples (bactériennes, mécaniques, généraux..) n'a à ce jour, aucun traitement reproductible et apportant une rémission totale à 100% des cas. Des traitements de la péri-implantite sont actuellement mis en œuvre avec des nouveaux protocoles. C'est avec l'évolution de la technologie médicale, que le laser fut utilisé pour la première fois en dentisterie dans les années 60. Certains sont utilisés en tant qu'adjuvants dans le traitement de la péri-implantite. Dans cette optique, une analyse de la littérature, portant sur le traitement des péri-implantites au laser est réalisée afin de proposer un nouveau protocole de thérapie photodynamique basé sur l'utilisation d'une nouvelle longueur d'onde, la 450 nm du laser diode bleu et comme photosensibilsant la curcumine, déjà décrite dans la littérature pour ses propriétés anti-inflammatoires et antimicrobienne.
