Connaissant déjà les différents déterminants de la prescription, nous avons voulu identifier les déterminants associés à un changement de prescription d'une anticoagulation orale. Méthode Nous nous sommes appuyés sur les données de l'étude CACAO, une étude de cohorte nationale, prospective, menée en 2014, réalisée auprès d'une population de patients sous traitement anticoagulant oral et bénéficiant d'un suivi par un médecin généraliste. Les données ont été collectées à l'aide d'un questionnaire auprès du médecin généraliste et d'un questionnaire électronique réalisé lors de l'inclusion de chaque patient. L'objectif est de décrire les facteurs associés au changement de prescription d'une anticoagulation orale. Résultats 406 médecins généralistes ont inclus 7 154 patients sous traitement anticoagulant oral. 5 699 patients éraient sous AVK et 1 455 sous AOD. Les facteurs associés à un changement de prescription sont : l'âge, le sexe féminin, l'absence d'antécédent d'IDM, un antécédent d'hémorragie hospitalisée, l'absence de traitement pas fibrate, avoir pour indication une FA, ne pas avoir pour indication une prothèse valvulaire .Discussion Notre échantillon de patients, semble être représentatif de la population générale des patients traités par anticoagulants oraux en France. Notre population de médecins est plus jeune, plus impliquée dans la recherche clinique et l'enseignement. Notre étude permet de mettre en évidence plusieurs facteurs représentatifs associés au changement de prescription d'une anticoagulation orale.

Ces médicaments présentent l'avantage d'une administration par voie orale et ne nécessitent pas de surveillance biologique, se rapprochant ainsi de l'anticoagulant idéal. Bien que dans la plupart des cas la surveillance biologique des AOD en routine ne semble pas justifiée, certaines situations cliniques particulières ou situations d'urgence nécessiteraient l'évaluation de l'effet anticoagulant. Le dosage spécifique des AOD repose sur la mesure de l'activité anti-Xa avec des calibrateurs spécifiques pour la surveillance du rivaroxaban et de l'apixaban et sur la mesure de l'activité anti-lla ou la réalisation d'un temps de thrombine dilué (Hemoclot®) pour la surveillance du dabigatran. Toutefois, ces tests en cours de standardisation sont peu disponibles dans les laboratoires. Il existe une solution dégradée reposant sur les tests d'hémostase de routine comme le taux de prothrombine ou le temps de céphaline avec activateur permettant uniquement d'orienter sur la présence ou non du médicament. A l'heure actuelle, les antidotes spécifiques des AOD sont en cours de développement mais ne sont pas encore commercialisés. De plus, aucune recommandation officielle des autorités de santé n'est disponible concernant la conduite à tenir dans les situations nécessitant une réversion de l'effet anticoagulant. Des groupes d'experts ont élaboré des propositions pour la prise en charge des hémorragies graves et la chirurgie urgente. Ainsi, en complément des traitements symptomatiques indispensables à tout saignement, l'administration de complexes prothrombiques activés ou non est actuellement proposée pour antagoniser l'effet anticoagulant de ces molécules. La formation des praticiens et l'éducation des patients seront indispensables pour une utilisation correcte de ces traitements afin de ne pas compromettre les progrès thérapeutiques de ces nouvelles molécules

Face à l'utilisation croissante des AOD, de nouvelles recommandations sont alors émises pour gérer les situations à risque susceptibles d'apparaître chez les patients traités par ce traitement. Une enquête a été réalisée dans deux officines de la Loire auprès de 20 patients traités par un AVK et de 18 patients traités par un AOD dans le but d'évaluer le niveau de connaissance du patient concernant son traitement anticoagulant par voie orale.

Bien qu’ils soient sélectifs et à grande fenêtre thérapeutique, ils présentent un risque hémorragique comparable aux antivitamines K (AVK). L’éducation thérapeutique pour les patients (ETP) traités par AVK permet une diminution des évènements hémorragiques. Pour ces raisons, il a été décidé de développer un programme d’ETP dans le service de cardiologie de l’hôpital Saint-Antoine pour les patients traités par un anticoagulant par voie orale. L’objectif de ce travail est de pouvoir formuler des objectifs thérapeutiques pertinents pour ce projet. Matériels et méthodes : 1/ Mise en place d’un groupe de travail multidisciplinaire. 2/ Elaboration d’un questionnaire anonyme pour déterminer les attentes de tous le soignants. Les questions portent sur le ressenti des soignants, sur les informations transmises, leur intérêt pour l’ETP, les modalités pratiques du programme (thématiques à aborder, intérêt d’aborder ou non la pathologie, patient seul ou famille, type de séances et d’outils à développer). Résultats : Une équipe multidisciplinaire a été constituée, elle est composée d’un pharmacien, d’un interne en pharmacie, d’un infirmier, d’un médecin. Le questionnaire a été distribué à l’ensemble des personnels du service de cardiologie de l’hôpital Saint-Antoine. L’analyse des réponses (n=41) a permis à l’équipe multidisciplinaire de formuler des objectifs éducatifs pertinents et d’élaborer un programme. Il s’articule en 1 diagnostic éducatif individuel, une séance individuelle et 3 évaluations des connaissances dont 2 téléphoniques. Un guide éducateur a été rédigé ainsi que 2 guides patients (1 guide pour le rivaroxaban et 1 guide pour le dabigatran etexilate), ils sont le pivot des séances d’ETP. Conclusion, perspectives : Le programme a débuté depuis peu de temps, la faisabilité est en cours d’évaluation et les résultats n’ont pas encore été analysés sur les premiers patients. Un prochain travail devra être mené afin d’analyser les résultats issus des évaluations des patients et de déterminer si les objectifs éducatifs fixés par ce programme ont été acquis par les patients.

Book excerpt: Les cinquante dernières années ont vu le règne des AVK, de l’héparine et des HBPM, mais ces médicaments sont un véritable défi pour le médecin quant à leur régulation et une source de plainte non négligeable par le patient. C’est pourquoi la première décennie du XXIème siècle a vu l’arrivée de nouveaux anticoagulants oraux ne nécessitant pas de monitoring. Le dabigatran étexilate et le rivaroxaban font partie de ces nouveaux anticoagulants dont le mode d’action repose, respectivement, sur l’inhibition directe de la thrombine et du facteur Xa. Ils ont été testés avec succès pour la prévention des accidents thromboemboliques veineux après une chirurgie pour une PTH ou une PTG et ont obtenu l’AMM en 2008 pour cette indication. Ces recherches ont servi de cadre pour des études visant à les évaluer dans la prévention des AVC chez les patients atteints de cardiopathies emboligènes. Ils ont montré leur non-infériorité dans des études de phase III chez des patients atteints de FA, et leur potentiel dans des études de phase II en prévention secondaire chez des patients ayant développé un SCA. Leur arrivée dans ces indications est espérée depuis longtemps, quelques points restent néanmoins à préciser, tels que l’absence d’antidote, le suivi de leur activité anticoagulante et la question du coût qui se révèle non négligeable et est surtout reliée à la prise en charge des effets indésirables. Ceux-ci proviennent principalement d’une mauvaise compréhension de la maladie (notamment des enjeux du traitement anticoagulant), montrant que l’éducation thérapeutique a une place majeure à prendre si l’on veut provoquer un changement quant aux données de santé publique.

Pour les actes à rique hémorragique faible à modéré, elle préconise le saut de laprise de l'AOD la veille au soir avant le geste et la reprise de l'AOD 24 heures après l'intervention. Pour les actes à risque hémorragiques élevé, un arrêt du traitement à J-5, pour une intervention prévue à J0 est proposé. Le traitement est ensuite repris le lendemain matin.

This volume is designed to present our experience with abdominal angiography. We deliberately refrained from any systematic discussion of the genitourinary tract, which has been adequately dealt with in the literature, also with respect to angiographic findings. Our interest in the retroperitoneal region is based mainly on its significance in differential diagnosis. In ten years of angiographic activity, our Department had made successful use of a simple technique which appears suitable also for smaller hospitals. We wish to point out its diagnostic potential and, at the same time, to outline its limitations. Our experience embraces 2804 abdominal angiograms, which we have classified according to clinical and morphologic anatomical criteria. Their diagnostic interpretation has been compared with the surgical or histopathological results. This may help others to avoid errors of the type which we discovered in our own work. Angiographic diagnosis requires not only familiarity with normal radiographic anatomy, but also specific knowledge of angiographic patho morphology. We have tried to identify those features which typify the individual findings and to derive therefrom valid generalizations with the aid of simple sketches.
