Book excerpt: Depuis la classification parodontale d'Armitage 1999, puis dans la nouvelle classification parodontale de Chicago 2017, les accroissements gingivaux font partie des maladies parodontales et sont classés parmi les maladies gingivales. L'accroissement gingival fait partie des atteintes fréquentes de la gencive. Ce dernier correspond à une augmentation des tissus mous, parfois importante, qui peut être à l'origine d'altérations fonctionnelles et esthétiques. Les trois parties de la muqueuse gingivale (gencive marginale, gencive attachée, papille interdentaire) peuvent être atteintes d'une augmentation de volume de manière localisée ou bien diffuse. On distingue les accroissements induits par la plaque de ceux qui ne sont pas induits par la plaque. Les étiologies sont diverses et variées mais les formes cliniques sont la plupart du temps assez ressemblantes. L'accroissement peut parfois être lié à des pathologies d'ordre général, ce qui peut rendre la démarche diagnostique plus complexe. Cette dernière est importante et fera appel aux multiples connaissances techniques, théoriques et cliniques du chirurgien-dentiste et elle reposera essentiellement sur l'interrogatoire médical du patient ainsi que sur un examen buccal approfondi, en association avec d'éventuels examens complémentaires. Le diagnostic orientera alors la gestion des lésions ainsi que les options thérapeutiques. L'objectif de ce travail sera de présenter les différentes étiologies et manifestations cliniques des accroissements gingivaux diffus, la démarche diagnostique ainsi que les conduites à tenir adaptées à chaque pathologie.

Ils sont une complication relativement fréquente des traitements orthodontiques par appareillages fixes multi-attaches. Au cours d'études antérieures, il nous est apparu que le biofilm dentaire, dans son aspect quantitatif, ne semble pas toujours être en relation avec le développement de l'accroissement gingival : il n'en serait donc pas toujours un facteur de risque. Au vu de ces constatations, nous avons poursuivi les investigations en créant et réalisant une étude préliminaire ayant pour but d'analyser l'évolution qualitative du microbiote infragingival présent au cours du traitement orthodontique, via des techniques microbiologiques de PCR quantitative. Cette étude a été réalisée durant un an et demi, au centre hospitalier universitaire d'odontologie de Nice, ainsi qu'au laboratoire Micoralis EA7354, Nice Sophia-Antipolis. L'ensemble des protocoles administratif, clinique, et d'analyse laboratoire ont été mis au point et optimisés. Ils ont de plus été testés et validés grâce à cette étude exploratoire, qui a été un test primordial avant mise en œuvre de cette étude à plus grande échelle, par augmentation du nombre de sujets inclus. D'autre part, de premiers résultats microbiologiques ont été obtenus, à partir de l'étude de 106 espèces bactériennes communément retrouvées dans la flore orale. Ceux-ci montrent des variations bactériennes très importantes au cours du traitement orthodontique, au niveau d'une gencive saine, ainsi que lors d'accroissements gingivaux existants en présence d'appareillages fixes multi-attaches. Ces variations sont également présentes entre chaque patient, entre chaque temps de l'étude, et entre chaque secteur dentaire étudié. Ainsi, tout est prêt pour mettre en œuvre l'étude à proprement parler, incluant cette fois un grand nombre de participants. Celle-ci est nécessaire afin de pouvoir confirmer, ou non, notre hypothèse de flore spécifique présente au niveau de ces accroissements gingivaux.

Dans un premier temps, une revue de la littérature a été effectuée. L'accroissement gingival est fréquent au cours du traitement orthodontique par multi-attaches, notamment chez les adolescents. Son importance augmente avec la durée du traitement et il est réversible après la dépose de l'appareil. Il est associé à une inflammation gingivale et à une quantité de plaque variable. Dans un second temps, une étude épidémiologique transversale exhaustive a été menée. L'accroissement gingival, l'indice de plaque de Löe et Siness et le biotype gingival ont été évalués lors d'un examen clinique. L'accroissement gingival a été déclaré présent à partir de quatre papilles affectées. Le statut socio-démographique et médical du patient, ses habitudes d'hygiène bucco-dentaire et les caractéristiques du traitement orthodontique par multi-attaches ont été recueillis. 193 patients ont été inclus dans l'étude et la moyenne d'âge était de 17,2 ± 7,8 ans. La prévalence d'accroissement gingival était de 64,8%. Les patients qui présentaient le plus d'accroissement gingival étaient de sexe masculin (p=0,118), en période pubertaire (p=0,072), et traités depuis plus d'un an (p=0,106) ; avec des attaches en métal (p=0,006), des ligatures et des chaînettes élastomèriques (respectivement p=0,045 et p=0,133). Après analyse multivariée, seules les attaches en métal constituaient un facteur de risque (IC 95% [0,964 ; 3,389] OR=3,010).

Cette thèse fait une synthèse des données actuelles de la science sur les définitions, étiologies, aspects cliniques et microbiologiques des accroissements gingivaux développés à la suite de traitements d'orthopédie dento-faciale. Les données de la littérature y sont rassemblées pour définir les facteurs associés à ces pathologies. La compréhension des mécanismes d'initiation et de développement des accroissements gingivaux ainsi que leur passage éventuel d'un état non inflammatoire à un état inflammatoire devrait permettre une meilleure prise en charge des patients atteints de ces pathologies, via une collaboration étroite entre le praticien chargé de l'orthopédie dento-faciale et celui chargé de la prévention buccodentaire.
