Book excerpt: In 2019, WHO and Coalition for Children Affected by AIDS convened a learning session of scientific and programmatic experts to consolidate the evidence on why HIV-affected adolescent mothers and their children are being left behind and to deliberate on the multiple-level changes needed to improve their outcomes. This technical brief follows on from that session and will be useful to HIV programme managers in health ministries and other adolescent- and youth-linked line ministries, especially those in in sub-Saharan Africa, in implementing, monitoring and evaluating adolescent and youth-responsive and -friendly health services for young mothers living with HIV. This technical brief aims to inform and support global dialogue and accelerate action on prioritizing services and support for adolescent and young mothers living with HIV. It details core programmatic examples and key strategies actions from across sub Saharan Africa that demonstrate how governments, health facilities, social services, communities, families and adolescent and young mothers are working together to bridge the gap between adolescent and adult-focused HIV and maternal health services. The programme examples provided serve to highlight potential and ongoing learnings in countries.

It also shows what social and economic barriers occur when mothers, or to some extend people in general, try to access anti-retroviral treatments (ART). As research has shown, factors like social stigma still play a large role why so many people are not seeking help. Sub-Saharan countries still have an unproportioned part of the global HIV infection compared to the rest of the globe. The most common form of getting infected by HIV is by heterosexual contact, followed by MTCT. Even with the slowly declining numbers of infected people and people with no access to treatments, this still is a big issue that should be tackled and done more research on, especially on the mother to child transmission of HIV. MTCT contributes largely to the number of children affected by HIV by the age of ten and under. The author intents to point out why this should be worked on more and why factors like education and reducing social stigma should be advanced in order to enable a faster decline of vertical transmission numbers. Furthermore, basic mechanisms of MTCT are going to be explained and how it possibly can be prevented in every stage of the pregnancy. Additionally, the author clarifies why social, cultural, and economic factors play such a big role in fighting against HIV. The paper aims to show, how all these factors tie together and what future research could be done in order to reduce the number of people getting affected by HIV, especially infants and young people.

It serves as an update to the previous edition of the consolidated guidelines on HIV. These guidelines continue to be structured along the continuum of HIV care. Information on new combination prevention approaches, HIV testing, ARV regimens and treatment monitoring are included. There is a new chapter on advanced HIV disease that integrates updated guidance on the management of important HIV comorbidities, including cryptococcal disease, histoplasmosis and tuberculosis. The chapter on general HIV care, contains a new section on palliative care and pain management, and up to date information on treatment of several neglected tropical diseases, such as visceral leishmaniasis and Buruli ulcer. New recommendations for screening and treating of cervical pre-cancer lesions in women living with HIV are also addressed in this chapter. Guidance on service delivery was expanded to help the implementation and strengthening the HIV care cascade. Importantly, this guidance emphasizes the need for differentiated approaches to care for people who are established on ART, such as reduced frequency of clinic visits, use of multi-month drug dispensing and implementation of community ART distribution. The adoption of these efficiencies is essential to improve the quality of care of people receiving treatment and reduce the burden on health facilities, particularly in resource limited settings.

Book excerpt: South Africa is reported to have the highest prevalence of HIV/AIDS infections in Sub-Saharan Africa. As a result the quality of life of families living with HIV/AIDS is negatively affected. Literature indicates that the majority of affected persons are young children and adolescents whose siblings or parents are infected with HIV/AIDS. Most affected adolescents are reported to have difficult social lives due to their parents illness, difficult financial situations at home, stigma and discrimination within the society. The study attempts to explore the experiences of adolescents in households where the mother is HIV/AIDS positive. This research project is an exploratory study using a narrative approach within a qualitative methodological design. The study was conducted with 6 Xhosa-speaking adolescent boys (2) and girls (4) aged 12 to 15 years. An open-ended interview schedule was used to prompt participants to tell their stories. Data was collected by means of a voice recorder in order for adolescents to tell their stories. The responses were transcribed verbatim, translated and verified with the participants. The data were analysed by means of narrative analysis. The results indicate that most adolescent participants were coping with the mother being HIV/AIDS positive and maintained hope for their futures. Adolescents were coping due to support from friends and relatives. Some adolescents experienced rejection and discrimination. The biggest challenge in the home was due to socioeconomic status. The study is intended to benefit the community in terms of making recommendations to social workers at NGOs and the government sectors in terms of strengthening the existing support programmes in the community.
