The authors discussed, from Nerval and Houellebecq to NDiaye and Lê, provide a refreshingly “balanced” picture of mental illness, presenting madness or depression as a contestatory, creative stance against often mind-numbing social, racial or consumerist conventions, while refusing to play down the inevitable difficulties accompanying this isolating condition. The “dialectic effect” referenced in the title of the collection extends not only to the dynamics at work within the volume itself, as the different contributions implicitly dialogue with one another, but equally to the reader of these essays, who is engaged throughout in the debates put forward.

Book excerpt: The development of psychiatry in the Middle East, viewed through the history of one of the first modern mental hospitals in the region. ʿAṣfūriyyeh (formally, the Lebanon Hospital for the Insane) was founded by a Swiss Quaker missionary in 1896, one of the first modern psychiatric hospitals in the Middle East. It closed its doors in 1982, a victim of Lebanon's brutal fifteen-year civil war. In this book, Joelle Abi-Rached uses the rise and fall of ʿAṣfūriyyeh as a lens through which to examine the development of modern psychiatric theory and practice in the region as well as the sociopolitical history of modern Lebanon. Abi-Rached shows how ʿAṣfūriyyeh's role shifted from a missionary enterprise to a national institution with wide regional influence. She offers a gripping chronicle of patients' and staff members' experiences during the Lebanese Civil War and analyzes the hospital's distinctive nonsectarian philosophy. When ʿAṣfūriyyeh closed down, health in general and mental health in particular became more visibly “sectarianized”—monopolized by various religious and political actors. Once hailed for its progressive approach to mental illness and its cosmopolitanism, ʿAṣfūriyyeh became a stigmatizing term, a byword for madness and deviance, ultimately epitomizing a failed project of modernity. Reflecting on the afterlife of this and other medical institutions, especially those affected by war, Abi-Rached calls for a new “ethics of memory,” more attuned to our global yet increasingly fragmented, unstable, and violent present.

Political thinkers reasoned that if government was to rest in the hands of individuals, then measures should be taken to understand the deepest reaches of the self, including the state of madness. Marcel Gauchet and Gladys Swain maintain that the asylum originally embodied the revolutionary hope of curing all the insane by saving the glimmer of sanity left in them. Their analysis of why this utopian vision failed ultimately constitutes both a powerful argument for liberalism and a direct challenge to Michel Foucault's indictment of liberal institutions. The creation of an artificial environment was meant to encourage the mentally ill to live as social beings, in conditions that resembled as much as possible those prevailing in real life. The asylum was therefore the first instance of a modern utopian community in which a scientifically designed environment was supposed to achieve complete control over the minds of a whole category of human beings. Gauchet and Swain argue that the social domination of the inner self, far from being the hidden truth of emancipation, represented the failure of its overly optimistic beginnings. Madness and Democracy combines rich details of nineteenth-century asylum life with reflections on the crucial role of subjectivity and difference within modernism. Its final achievement is to show that the lessons learned from the failure of the asylum led to the rise of psychoanalysis, an endeavor focused on individual care and on the cooperation between psychiatrist and patient. By linking the rise of liberalism to a chapter in the history of psychiatry, Gauchet and Swain offer a fascinating reassessment of political modernity.

Drawing on the accumulated knowledge of a lifetime devoted to the study of the individual in modern democratic society, Ehrenberg shows that the phenomenon of modern depression is not a construction of the pharmaceutical industry but a pathology arising from inadequacy in a social context where success is attributed to, and expected of, the autonomous individual. Nietzsche situates it in the long history of producing an animal able to promise. Its returns in the 20th century allow us to grasp the connection between phenomenology's exploration of passivity, followed by interpretations of the human reality in a world and open to a call that it can hardly assume. The study thus begins with Kant; it probes late Idealism and Romanticism, the metaphysical vitalism that flickered with Schopenhauer, the aesthetics and religious senses of *Angst* in Nietzsche and Kierkegaard. It turns to three avatars of anxiety in the evolving psychoanalysis before exploring the return to rationalism and formalism in 20th century phenomenology, followed again by efforts to resituate human beings in world and body as well as, significantly, before the anxiogenic "other""--
