Bien que considérée comme une médecine naturelle, la pratique de l'aromathérapie à des fins thérapeutiques n'est pas sans risque. La toxicité des HE est de plus en plus décrite dans la littérature scientifique et on observe chaque année, un nombre grandissant d'événements indésirables imputables aux HE. Une enquête a été réalisée auprès de 330 professionnels officinaux afin de mieux connaître le rôle du pharmacien d'officine dans l'optimisation et la sécurisation de l'aromathérapie. Ce dernier apparaît être, par sa formation et par sa responsabilité quant à la qualité, la sécurité et l'efficacité des HE, un professionnel de santé impliqué. Il joue un rôle important dans la prévention et dans la gestion des risques et notamment des risques liés à l'utilisation des HE. La prévention des évènements indésirables est une de ses compétences, par son rôle de conseil, d'accompagnement et de sécurisation de la dispensation. La gestion des évènements indésirables est également une de ses missions, en orientant le patient vers une prise en charge adaptée, mais également en déclarant l'événement indésirable auprès des organismes de vigilance. Par la multitude de statuts auxquels les HE peuvent répondre, il n'existe pas aujourd'hui de système de recueil et d'analyse d'évènements indésirables impliquant toutes les HE et de ce fait les déclarations ne sont pas centralisées, ce qui ne permet pas d'avoir une vision globale de ces évènements indésirables. En parallèle des organismes institutionnels en place, les pharmaciens peuvent être à l'initiative d'une Aromatovigilance, c'est à dire d'un système de recueil et d'analyse des évènements indésirables impliquant toutes les HE. Celui-ci pourrait venir renforcer l'exhaustivité des données des différents organismes de vigilance. L'association AROMASECURE, créée par des professionnels titulaires d'un DU d'aromathérapie, propose un système informel d'Aromatovigilance. Ces systèmes sont encore trop peu connus des professionnels et pourraient améliorer la visibilité sur les évènements indésirables impliquant les HE.

Elles peuvent provoqués des malformations congénitales et même, entre autres, concourir au processus de cancérogénèse. Certaines, difficiles à utiliser à cause de leur caractère fortement toxique sont interdites à la vente en dehors des officines et doivent être délivrées sur ordonnance du médecin. L'aromathérapie, loin d'être anodine, nécessite la plus grande précaution d'usage et de ce fait, le conseil éclairé d'un professionnel de santé. Le pharmacien d'officine a un rôle primordial dans la prévention des risques inhérents à une mauvaise pratique de l'aromathérapie. Le conseil associé à la délivrance d'huiles essentielles est la seule garantie qu'a le consommateur d'utiliser correctement une huile essentielle.
