Book excerpt: Les maladies parodontales sont des maladies inflammatoires chroniques dues à une infection causée par des bactéries dites parodontopathogènes. Face aux attaques bactériennes les leucocytes polymorphonucléaires produisent des espèces réactives de l’oxygène (ERO) pour s’en protéger. Cependant, la production excessive d’ERO a un effet délétère sur les cellules et tissus de l’hôte lorsque le système de défense antioxydant n’est pas suffisamment efficace pour les neutraliser ce qui aboutit au phénomène de stress oxydatif. De nombreuses études ont montré que le stress oxydatif joue un rôle clé dans l’inflammation parodontale, c’est pourquoi les thérapies antioxydantes présentent un intérêt certain dans le traitement des maladies parodontales. En plus des antioxydants enzymatiques, l’alimentation est une source abondante d’antioxydants qu’on retrouve notamment dans les fruits et les légumes. En effet, les vitamines C, E, A, certains pigments alimentaires ainsi que le thé présentent un potentiel antioxydant qui a fait l’objet d’investigations concernant leurs liens avec les maladies parodontales. L’objectif de cette thèse est d’élucider, à travers l’implication du stress oxydatif et de la nutrition dans la pathogénèse des maladies parodontales, le rôle des antioxydants alimentaires dans les stratégies préventives et thérapeutiques de ces maladies.

Ceci avant de réaliser un aparté sur les conséquences parodontales des troubles du comportement alimentaire. L'étude de ce lien entre l'apport nutritif et la santé parodontale est un pas de plus vers la compréhension des pathologies multifactorielles que sont les maladies parodontales. L'objectif étant d'améliorer la prise en charge de notre patient, dans sa globalité. Tout l'enjeu de ce travail est de questionner la pertinence de la prescription de complémentation(s) nutritionnelle(s) en parodontologie.

Book excerpt: Une étude observationnelle menée au CHU de Toulouse entre 2019 et 2021, a permis de recueillir des données anthropométriques, médicales, de stress et de nutrition de plus de 500 sujets avec pour objectif de mieux définir les déterminants de la santé parodontale. A partir de ces données, un modèle prédictif a été élaboré par apprentissage machine et l'explicabilité globale générée. Les données montrent que la nutrition et l'indice de masse corporelle sont des déterminants cruciaux de la prédiction de la parodontite. Outre le fait que cela renforce le lien supposé entre maladie parodontale et obésité, ce modèle peut être vu comme un nouvel outil susceptible d'améliorer la prévention dans le domaine de la santé parodontale et d'apporter de nouvelles orientations thérapeutiques.
