Book excerpt: Introduction : Le but de l'étude était d'évaluer la position sociale actuelle vis-à-vis de l'allaitement maternel, ainsi que le degré de connaissance sur ce sujet auprès d'un échantillon de la population générale française. Méthodologie : L'étude était observationnelle, transversale à visée descriptive, réalisée sur un échantillon de 3 259 personnes qui utilisent Internet et les réseaux sociaux. Le recueil de données a débuté le 27 juin 2018 et a pris fin le 23 septembre 2018. Résultats : 60% des personnes considèrent que la tendance sociale actuelle est plutôt en faveur de l'allaitement maternel. La majorité des femmes ayant déjà allaité déclarent que leur entourage était soutenant face à leur choix d'allaiter. 36,93% des professionnels et 32,05% des non-professionnels pensent que l'allaitement exclusif signifie que le bébé peut boire de l'eau en plus du lait maternel. 74,41% des professionnels et 73,71% des non-professionnels sont convaincus que toutes les femmes en bonne santé ne sont pas capables d'allaiter. Conclusion : La perception sociale concernant l'allaitement maternel est positive, cependant les connaissances sur le sujet demeurent insuffisantes, non seulement chez les non-professionnels mais également chez les professionnels. Il faudrait éduquer la population par le biais du système éducatif, de campagnes médiatiques faites par les institutions du système de santé français et d'autre part créer un référent en allaitement, uniformiser et multiplier les formations sur l'allaitement maternel des différents professionnels de santé.

Book excerpt: La France apparaît au premier rang pour la performance globale de son système de santé, cependant il existe des inégalités sociales de santé y compris dans le domaine de la périnatalité. Les bénéfices de l’allaitement étant reconnus pour la santé de la mère et de l’enfant, nous avons souhaité effectuer une revue de la littérature, afin d’évaluer les inégalités sociales de comportement vis-à-vis de l’allaitement en France et dans les autres pays développés. Pour cela, deux équations de recherche ont été réalisées sur les bases de données Medline, Cochrane et la Banque de données en Santé Publique. 17 études sur 15 pays ont été recensées pour la prévalence de l’allaitement dans la population générale et 22 études pour 13 pays dans les sous-groupes socialement différenciés. Tous les articles ont été sélectionnés par deux personnes différentes. Dans la population générale, la France se situe dans les pays qui ont les taux les plus bas d’initiation de l’allaitement. Pour le maintien de l’allaitement, elle se situe en dernière position. Les catégories de femmes vulnérables vis-à-vis de l’allaitement maternel sont les mères jeunes isolées socialement et vivant dans un quartier défavorisé. Le contexte économique est également source d’inégalité sociale, notamment le faible revenu, le bas niveau d’études, l’appartenance à une catégorie professionnelle basse et le fait de ne pas avoir d’emploi. Chez les femmes d’origine étrangère ou migrantes, il faut favoriser leur propension naturelle à l’allaitement maternel et les protéger du phénomène d’acculturation.

En effet, l'allaitement maternel comporte des difficultés dans son initiation, sa prolongation et son arrêt car l'influence de l'environnement est très présente. Pour autant, une femme sur cinq franchit ces difficultés et perdure son allaitement maternel pour une durée qui, la plupart du temps, est indéterminée. Nous nous sommes interrogés : Pourquoi certaines femmes font-elles le choix de prolonger leur allaitement au-delà de six mois et comment vivent-elles cette expérience ? Grâce à dix témoignages de femmes ayant allaité plus de six mois et dix ayant allaité plus de deux ans, nous avons pu conclure qu'elles font ce choix afin de faire perdurer les bienfaits nutritifs et surtout relationnels de l'allaitement maternel. Mais elles sont confrontées à des difficultés d'ordre anatomique, pédiatrique et principalement d'ordre social auprès de : leur conjoint, leur entourage, leurs collègues de travail, des professionnels de santé et d'inconnus. Cette étude nous a permis notamment de découvrir comment les femmes ont su surmonter les difficultés qu'elles ont rencontrées. Cette expérience permettra aux sages-femmes et aux autres professionnels de santé, d'accompagner au mieux les femmes souhaitant allaiter, en leur apportant des conseils adaptés sur la prise en charge et la durée de l'allaitement maternel.

Book excerpt: Ce mémoire de maîtrise en sociologie vise à identifier les représentations sociales de l'allaitement maternel au Québec du point de vue de femmes, enceintes de leur premier enfant, qui désirent allaiter. Afin de répondre aux questions de cette recherche, des informations ont d'abord été recueillies sur le contexte de la prise de décision d'allaiter des femmes afin d'éclaircir leurs motivations personnelles, mais aussi l'influence de leurs réseaux personnel (famille, conjoint, amis) et professionnel en regard de ce choix. Dans un deuxième temps, les femmes ont été longuement interrogées sur leurs conceptions des représentations sociales du lait maternel et du geste d'allaiter (terminologie utilisée; le sens, les valeurs et les normes sociales accordés à cette pratique; les difficultés; le rôle de mère; le rapport au corps et à la sexualité durant la période d'allaitement; l'allaitement dans l'espace public, etc.). Pour répondre aux objectifs de cette recherche, une méthode qualitative inductive et exploratoire a été privilégiée: l'analyse par théorisation ancrée. Des entretiens en profondeur ont été réalisés auprèS de onze (11) femmes québécoises, âgées entre 25 et 35 ans, en période prénatale. Le contenu des entrevues a fait l'objet d'une analyse thématique et conceptuelle, verticale et horizontale. Parmi les résultats obtenus, on remarque que le lait maternel et le geste d'allaiter sont représentés positivement par les femmes interrogées qui les décrivent principalement en des termes avantageux. En accord avec le discours institutionnel de santé publique, les répondantes considèrent l'allaitement maternel comme un geste " allant de soi " et " responsable " qui apporte des bénéfices pour la mère et pour l'enfant sur les plans physiques, psychologiques, affectifs et cognitifs. Ce geste est jugé " incontournable " dans la mesure où il est associé à la représentation que l'on se fait d'une " bonne mère ". ________________________________________________________________

De plus, malgré les nombreux bénéfices et recommandations ce sont les femmes en situation de précarité qui ont le taux d'allaitement le plus bas. Méthode : Il s'agit d'une étude qualitative, basée sur des entretiens semi-dirigés auprès de dix femmes en situation de précarité de la région lyonnaise. La totalité des entretiens a été retranscrite et nous avons réalisé une analyse transversale des données.Résultats : Le choix de l'allaitement répond à de multiples facteurs. L'origine ethnique et culturelle est un facteur déterminant. De plus, même si elles considèrent le lait maternel comme supérieur aux préparations pour nourrissons comme en témoignent les nombreux avantages qu'elles lui attribuent, elles en pondèrent cependant les bénéfices devant les freins et les difficultés rencontrés. Ces derniers ne sont pas spécifiques, mais sont parfois suffisants pour compromettre la poursuite de l'allaitement. Ainsi, c'est le soutien reçu qui va faire la différence. Elles le trouvent spontanément auprès de leur entourage familial et amical. Les professionnels de santé ne sont sollicités qu'en deuxième intention et n'offrent pas pleine satisfaction. Leur premier recours reste les centres de Protection Maternelle et Infantile pour leur proximité, leur facilité d'accès et leur équipe pluridisciplinaire.Conclusion : C'est sur la qualité de l'information délivrée avant l'accouchement, les pratiques hospitalières et le réseau de soutien social qu'il faut œuvrer pour améliorer le taux d'allaitement dans cette population.

Calhoun, detail the vulnerability to being duped and deceived and the difficulty of cooperating when values differ. Murray Bowen, M.D., the first researcher to observe several live-in families, for up to three years, at the National Institute of Mental Health. Describing how family members overly influence one another and distribute stress unevenly, Bowen described both how symptoms and family leaders emerge in highly stressed families. Our brain is not organized to automatically perceive that each family has an emotional system, fine-tuned by evolution and "valuing" its survival as a whole, as much as the survival of any individual. It is easier to see this emotional system function in ants or mice but not in humans. The emotional system is organized to snooker us humans: encouraging us to take sides, run away from others, to pressure others, to get sick, to blame others, and to have great difficulty in seeing our part in problems. It is hard to see that we become anxious, stressed out and even that we are difficult to deal with. But "thinking systems" can open the doors of perception, allowing us to experience the world in a different way. This book offers both coaching ideas and stories from leaders as to strategies to break out from social control by de-triangling, using paradoxes, reversals and other types of interruptions of highly linked emotional processes. Time is needed to think clearly about the automatic nature of the two against one triangle. Time and experience is required as we learn strategies to put two people together and get self outside the control of the system. In addition, it takes time to clarify and define one's principles, to know what "I" will or will not do and to be able to take a stand with others with whom we are very involved. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others. The effort involved in creating your Mindful Compass enables us to perceive the relationship system without experiencing it's threats. The four points on the Mindful Compass are: 1) Action for Self, 2) Resistance to Forward Progress, 3) Knowledge of Social Systems and the 4) The Ability to Stand Alone. Each gives us a view of the process one enters when making an effort to define a self and build an emotional backbone. It is not easy to find our way through the social jungle. The ability to know emotional systems well enough to take a position for self and to become more differentiated is part of the natural way humans cope with pressure. Now people can use available knowledge to build an emotional backbone, by thoughtfully altering their part in the relationship system. No one knows how far one can go by making an effort to be more of a self-defined individual in relationships to others. Through increasing emotional maturity, we can find greater individual freedom at the same time that we increase our ability to cooperate and to be close to others.
