Book excerpt: When the Board of Directors of the Belgian Ophthalmological Soci ety, in its session of November 26th 1978, asked me to prepare a report on strabismus to be presented at the joint meeting of the Dutch and Belgian Ophthalmological Societies to be held on June 13th 1981, I felt greatly honored but still more overwhelmed by the immensity of the task. I took advantage of the complete liberty given to me by the Board of Directors, first to limit the work to one particular form of strabismus, i.e. the convergent comitant form; second, to seek the help of what I thought to be the best strabologists in the Low Countries; third, to aim not at an encyclopedic treatise but at a practical volume destined to the general ophthalmologist. This volume is thus limited to the various aspects of convergent strabismus, more accurately of comitant convergent strabismus. The omission of the word" comitant" is purposely made to avoid the dif fic'ulties accompanying the explanation of this term and all the acroba tics needed to explain that most comitant strabismus are not complete ly comitant. The choice of this particular form of strabismus seems logical. First of all, it is the most common form of strabismus. On the other hand, most principles concerning examination and treatment can with some modifications be applied to other forms of strabismus.

After an analysis of the neurophysiology and neuropsychology of touch, exploratory manual behaviors, intramodal haptic (tactual-kinesthetic) abilities and cross-modal visual-tactual coordination are examined in infants, children and adults, and in non-human primates. These studies concern both sighted and blind persons in order to know whether early visual deprivation modifies the modes of processing space and objects. The last section is devoted to the technical devices favoring the school and social integration of the young blind: Braille reading, use of raised maps and drawings, sensory substitution displays, and new technologies of communication adapted for the blind. The monograph describes assessment and intervention approaches designed to promote very young children's self-regulation and adaptive behavior, and discusses new directions and outstanding questions in basic and applied research. Chapter 1 describes the modalities through which infants and toddlers receive sensory input from the environment and their bodies, and introduces the concept of sensory integration, which involves organizing sensation for adaptive use. Chapter 2 provides a framework for understanding how the sensory integrative process is revealed in the behavior of infants and young children, particularly with respect to arousal, attention, affect, and action. Chapter 3 introduces the concepts of sensory modulation and praxis, and introduces several frameworks for classifying problems in sensory integration and self-regulation. Chapter 4 presents guidelines and methods for the screening and assessment of sensory integration, and discusses interpretation of assessment data, leading to intervention planning. Chapter 5 addresses the needs of parents, the importance of modifying the sensory environment, and the nature of clinical reasoning during direct intervention; this chapter also provides intervention guidelines with accompanying case studies for children with hyperreactivity, hyporeactivity, and dyspraxia. As a first year resident, I was fortunate to encounter the series of articles by N. Lauge-Hansen and T. Baek Kristensen that classified ankle injuries according to their mechanism. I found this information quite helpful throughout my residency and early years of practice. Several years ago, an attempt was made to summarize this material for the benefit of the orthopaedic house staff of Thomas Jefferson University. It quickly become obvious that such a "manual" would require a great deal of professional illustration and editorial assistance in order to be effective. Almost simulta neously, a fortuitous encounter with Ms. Marie Low (at that time Medical Editor of Springer-Verlag New York Incorporated) provided both the stimulus and the means to present this information in the manner and detail which it deserved. Contributors were carefully selected for their knowledge and experience in particular areas and for their willingness to cooperate in providing a smooth flowing manuscript. This text contains little new material. Rather, it represents an attempt to bring together, under one cover, the wealth of extant information on this subject. Wherever a consensus could not be perceived, conflicting views have been sum marized as objectively as possible. Unfortunately, there are still many aspects of ankle injury for which we have more questions than answers.
