Elle se caractérise par des troubles cognitifs et comportementaux, notamment des troubles lexico-sérnantiques pouvant être sélectifs à une catégorie. Mais qu'en est-il pour les mots à valence émotionnelle? Les principales études ayant été effectuées sur les émotions dans la MA concernent la reconnaissance faciale des émotions, ne prenant en compte que l'aspect visuel. A ce jour, le traitement sémantique des noms émotionnels chez le patient Alzheimer n'a pas été étudié.Notre protocole consiste à présenter des séquences de films à valence émotionnelle et neutre, mettant en jeu la modalité auditive et la modalité visuelle. Les émotions que nous avons choisi d'étudier sont les émotions primaires, à savoir: la joie, la peur, la tristesse, la colère, le dégoût et la surprise. A la suite de ces séquences, le patient doit dénommer ou désigner, le cas échéant, l'émotion correspondante, et en donner des synonymes. D'autre part, la perception et le ressenti ont été évalués de manière subjective. Cette recherche a été réalisée sur une population de sujets sains, de sujets MCI, et de sujets Alzheimer. Cette étude a révélé un manque du mot des émotions à un stade précoce et ce, avant même la phase dite de démence. On a pu observer que la joie et la peur qui sont des émotions primitives, ont été dénommées avec facilité quel que soit le stade. En revanche, le dégoût et !a surprise seraient les émotions les moins bien dénommées du fait de leur aspect plus intellectuel. En ce qui concerne la fluence émotionnelle les résultats sont significatifs et montre bien qu'au plus le patient est à un stade avancé de la MA au moins il est capable de produire des synonymes.

Le manque du mot correspond à l'impossibilité ou difficulté momentanée à aller « récupérer » un mot connu que le locuteur est certain de connaître, en ayant le sentiment de l'avoir « sur le bout de langue ». Voilà une situation que tout un chacun a déjà vécue. Pourtant, chez certains patients ce manque du mot représente un symptôme prédominant de leur pathologie. C'est effectivement le cas des personnes ayant subi un accident vasculaire cérébral ainsi que des personnes atteintes de la maladie d'Alzheimer. Cette frustration de ne pas réussir à dire le mot que l'on souhaite, est une plainte quasi unanime de ces patients. Aussi, ce manque du mot ne s'exprime pas toujours de la même façon dans les pathologies. C'est pourquoi il est important de l'analyser précisément et d'identifier les mécanismes cognitifs altérés afin d'orienter du mieux possible le travail thérapeutique. Dans cette perspective, nous avons fait passer un test de dénomination à des patients aphasiques et des patients atteints de la maladie d'Alzheimer afin de mettre en évidence le manque du mot et d'analyser plus précisément ses différentes formes d'expression. Alors que nous pensions trouver des résultats discordants, en dénomination, entre les deux pathologies, nous avons constaté qu'ils n'étaient finalement pas si dissemblables. Certaines formes d'expressions du manque du mot semblent effectivement plus spécifiques à une pathologie qu'à une autre mais l'efficacité de chaque aide, quant à elle, présente des résultats quasi similaires.

Dans notre étude, nous avons recherché une corrélation entre ces troubles langagiers et olfactifs, puis nous avons affiné ce lien par critère de catégorie et de sous-catégorie, afin de mieux comprendre l'organisation cérébrale dans les processus sémantiques et olfactifs. Nous avons présenté des épreuves de dénomination lexicale, de fluence catégorielle, et d'identification olfactive, à neuf patients atteints de la maladie d'Alzheimer. Nos résultats ont mis en évidence une corrélation entre le manque du mot et le défaut d'identification olfactive. Plus précisément, nous avons observé une atteinte spécifique de la dénomination dans la catégorie biologique, que nous n'avons pas retrouvé en olfaction. Enfin, nous avons observé une corrélation entre ces deux troubles dans la catégorie biologique, mais pas dans la sous-catégorie les fruits. Ainsi, les troubles olfactifs et langagiers, qui présentaient déjà des corrélats cérébraux, ont révélé par leurs corrélations cliniques des similarités de fonctionnement et de traitement. L'organisation cérébrale sémantique serait établie selon les concepts et leurs réseaux, expliquant pourquoi nous n'avons pas retrouvé de corrélation pour la sous-catégorie de fruits. Notre étude a mis en évidence les liens entre mémoire sémantique et olfactive et interroge sur l'intérêt d'une amorce olfactive dans le cadre d'une prise en charge orthophonique du manque du mot.
