Book excerpt: Évaluer le devenir des patients partis sans attendre (PSA) la prise en charge au service d’accueil des urgences adultes, en terme d’évolution du motif initial de consultation et de parcours de soins ; établir leurs caractéristiques. Du 23 octobre 2014 au 04 janvier 2015, nous avons réalisé une étude observationnelle prospective sur le SAU de l’hôpital de Villefranche-sur-Saône. Ont été inclus tous les PSA majeurs enregistrés, non alcoolisés et partis avant tout début de prise en charge. Un questionnaire standardisé leur était soumis lors d’une procédure de rappel téléphonique entre J3 et J10 après leur passage. Durant cette période, 254 patients sont partis sans attendre sur 7173 passages, soit 3,5%. 119 (52%) ont pu être joints sur les 229 PSA inclus. 75% des répondants ont vu leur motif de consultation initial évolué vers la guérison ou l’amélioration. 64,7% ont consulté une autre expertise médicale suivant leur passage au SAU et ce, de manière précoce. 26,9% ont eu recours à leur médecin généraliste. Les caractéristiques démographiques du PSA sont les suivantes : jeune, consultant de sa propre initiative (60%), pour un motif médical (74%), de faible gravité. Le temps d’attente est la principale raison évoquée de départ prématuré (69%). Le PSA utilise de manière inadéquate les services d’urgence et contribue à leur encombrement. MATÉRIEL ET METHODES : Nous avons réalisé une étude prospective observationnelle cas témoins, sur le SAU du CHU Poitiers entre le 2 janvier et le 24 février 2012 et du CH La Rochelle entre le 11 juin et le 24 juillet 2012. Ont été inclus tous les « psa » majeurs enregistrés et non déments confus ou alcoolisés. Pour chaque « psa », un ou deux témoins étaient appariés selon un motif identique et de même gravité et selon des caractéristiques démographiques et temporelles similaires. Un suivi téléphonique par questionnaire standardisé était opéré à J2 puis à J15. Les caractéristiques démographiques et temporelles étaient relevées grâce au logiciel informatique du service. RESULTATS : 176 « psa » dont 57% de répondants au rappel téléphonique et 208 témoins dont 67% de répondants ont été inclus. En terme d'évolution du motif de consultation la différence est non significative entre les 2 groupes (p=0,7). La tendance est à un recours précoce à une autre source de soins (MG, clinique) dans le groupe « psa » (61%) par rapport au groupe témoin (39%) (p
