Book excerpt: Introduction : Dix ans après la publication des recommandations de bonnes pratiques par l'HAS en 2005, les troubles sensoriels de l'enfant restent encore insuffisamment dépistés et souvent détectés tardivement par les médecins généralistes. Ce sont pourtant les principaux acteurs du dépistage de l'enfant. L'objectif principal était d'explorer les écueils de ce dépistage à travers l'étude des pratiques et du ressenti des généralistes sur la faisabilité de ces examens chez l'enfant de 0 à 6 ans. Méthode : Une étude qualitative basée sur des entretiens semi-directifs individuels a été menée auprès de 20 médecins généralistes exerçant en Ille-et-Vilaine, recrutés selon un échantillonnage raisonné. La saturation des données a été obtenue. Une analyse thématique a été réalisée. Résultats : Il en résultait une grande hétérogénéité des pratiques. Les médecins attachaient de l'importance à leur rôle de prévention. Bien qu'ils estimaient ces examens réalisables sous certaines conditions, ils jugeaient leur pratique approximative et subjective. Le manque de formation, de temps et de matériel adapté étaient les principaux obstacles rencontrés. D'autres difficultés concernaient également les médecins les plus sensibilisés à cette pratique : la faible performance et les difficultés d'interprétations des tests ainsi que le problème d'accès à la filière d'aval. Ces freins renforçaient leurs doutes sur la réelle utilité de leur dépistage. Conclusion : Il semble qu'une clarification des recommandations et de certains items du carnet de santé, ainsi qu'un renforcement de la formation initiale soient nécessaires. Au vu de la situation démographique médicale actuelle, le transfert raisonné de compétences vers les paramédicaux spécialisés à des âges clés, et la participation des parents dans ce dépistage sont des solutions à considérer.

Ces dysfonctionnements sont reconnus dans plusieurs classifications diagnostique sous le terme de trouble de la régulation sensorielle. Ils sont observés fréquemment chez les patients TSA mais aussi dans d'autres troubles neurodéveloppementaux. A l'heure actuelle, la prévalence des Troubles de la Régulation Sensorielle chez les enfants consultant en pédopsychiatrie n'a jamais été évaluée, bien qu'elle semble être une population à risque. Objectifs : évaluer la prévalence des troubles de la régulation sensorielle chez les enfants entre 3 et 6 ans primo consultant en pédopsychiatrie à Marseille. Déterminer si certains motifs de consultation sont corrélés à la présence de trouble de la régulation sensorielle. Méthodes : une étude observationnelle prospective multicentrique a été menée pendant 4 mois à Marseille. Les troubles de la régulation sensorielle ont été évalués par la deuxième version du profil sensoriel court de Dunn. Résultats : cinquante-huit patients ont été inclus. Une différence significative a été trouvée entre la prévalence de TRS dans notre population de primo consultant en pédopsychiatrie et la population générale. Les analyses en sous-groupe selon le motif de consultation montrent qu'il y aurait davantage de TRS dans les sous-groupes Instabilité motrice/Inattention, Trouble des conduites, Trouble de la socialisation. Conclusion : nous avons observé une prévalence plus élevée des TRS dans la population consultant en pédopsychiatrie. Les TRS pourraient expliquer, ou au moins participer en partie, la symptomatologie des enfants. Les résultats sont en faveur d'un dépistage précoce des TRS dans la population des enfants consultant en pédopsychiatrie. L'évaluation des profils sensoriels de ces enfants permet une meilleure compréhension du fonctionnement de l'enfant et un ajustement des stratégies d'intervention et de soutien proposées à la famille.
