Le développement du langage est la résultante de multiples facteurs (génétiques, neurocognitifs, psychoaffectifs, sociaux...) hautement intriqués entre eux, ce qui rend leur compréhension complexe aux niveaux étiologique et épistémologique. La neuropsychologie cognitive a développé le concept de troubles spécifiques, qui par définition, n'ont pas comme cause un trouble psychopathologique, mais en sont fréquemment accompagné. En pratique, les troubles du langage nécessitent donc une évaluation multidisciplinaire et respectueuse de l'unité du développement de l'enfant. Sous l'impulsion des associations de parents d'enfants présentant des troubles spécifiques, des centres référents pour les troubles du langage et des apprentissages ont été créés à partir de 2001, dont le Centre du Langage de l'Hôpital Avicenne (CDL), individualisé au sein du service de psychopathologie de l'enfant et de l'adolescent. L'objectif de notre étude est d'évaluer l'effet d'un bilan dans le parcours de soin des enfants ainsi que sur les représentations parentales. Les résultats montre tout d'abord les spécificités de la population reçue (N=75) : l'importance du taux d'enfants de migrants (63%) (et de bilinguisme), un niveau socio-économique bas, une fréquence élevée de troubles psychopathologiques (80%) et une fréquence moindre de troubles spécifiques (32%). Les entretiens avec les parents retrouvent globalement une bonne compréhension du diagnostic, et une bonne satisfaction, particulièrement vis-à-vis des explications sur le fonctionnement psychique de leur enfant. Soixante dix pourcent estiment que le bilan a changé leur vision des troubles de leur enfant, le plus souvent d'une façon qui les a rassurés et leur a permis d'avoir une meilleure relation avec leur enfant. Les soins préconisés ont été initiés et perdurent un an après la fin du bilan dans deux tiers des cas. Les soins orthophoniques sont presque toujours en place; les soins psychologiques, individuels ou familiaux, ne le sont pas dans un tiers des cas. Enfin, les facteurs migration et compréhension du diagnostic sont corrélés positivement à la mise en place des soins (à la limite de la significativité). Par contre, les enfants présentant un trouble limite sont moins en soin que les autres, ces situations semblent très sensibles à la dissociation entre lieu d'évaluation et lieu de soin, vécue comme une discontinuité. Malgré ses limites, cette étude a ouvert plusieurs pistes d'évolution pour le CDL, au niveau des évaluations diagnostiques et des modalités de relais avec les soins ultérieurs.

The answers given to these qu- tions have deepened the already existing gap between philosophy and the history and practice of science. While the positivists argued for a spontaneous, steady and continuous growth of scientific knowledge the post-positivists make a strong case for a fundamental discontinuity in the development of science which can only be explained by extrascientific factors. The political, social and cultural environment, the argument goes on, determine both the questions and the terms in which they should be answered. Accordingly, the sociological and historical interpretation - volves in fact two kinds of discontinuity which are closely related: the discontinuity of science as such and the discontinuity of the more inclusive political and social context of its development. More precisely it explains the discontinuity of the former by the discontinuity of the latter subordinating in effect the history of science to the wider political and social history. The underlying idea is that each historical and - cial context generates scientific and philosophical questions of its own. From this point of view the question surrounding the nature of knowledge and its development are entirely new topics typical of the twentieth-century social context reflecting both the level and the scale of the development of science.
